DIRECT COMPOSITE VENEERS DR.SHUJA ASLAM ASSISTANT PROFESSOR(OPERATIVE DENTISTRY)

Indirectly fabricated ceramic veneers are the gold standard for esthetics and longevity; however, direct resin composite veneers are alternative

INDICATIONS Discoloration Enamel defects Diastema Malposed teeth like lingually or facially placed teeth Short teeth

Contraindications Insufficient tooth for bonding Bruxism

advantages Conservative(little or no enamel is removed) One visit as compared to indirect ceramic Cost effective No lab fees Not cause wear of opposing dentitions as ceramic restorations does Can be repaired

disadvantages Time taking Need clinical experience and operator skill Do not maintain their apperance long term like ceramic(may discolor)

Technique Shade selection Place gingival retraction cord to retract the gingiva or rubber dam Remove smallest amount of tooth structure if required Etch Wash and dry Place clear plastic strip and wedge interproximally Apply adhesive resin and light cure

Place resin composite, adapt and contour the material and then light cure in increments Contour the gingival margins and remove flash with 12 or 12B scalpel blade Check occlusion and do finishing with finishing burs Finish and polish with disks, polishing points Apply the rebonding resins(surface sealer) to restoration and margins Direct resin composite veneers can be produced by using single opacity shades or by layering various shades and opacities to obtain polychromatic esthetic restorations according to adjacent teeth, desired results, and patient expectations

If a tooth shows a dark discoloration, an opaque layer or masking agent is placed over the adhesive and polymerized When necessary, white, orange, or brown tints may be used to add a natural look to cervical or proximal surfaces of teeth or to internally characterize a restoration Translucency, the appearance of lobes or mamelons , and/or white or bluish characterization of the incisal edge may be added in the incisal third of the composite veneer.

Like class 4 restorations, wax up can also be done for veneers on models to show patients A custom matrix made from the wax-up, similar to the one described for a Class 4 restoration, can be fabricated from the wax-up to aid with placement of the veneers.
