MEANING Discharge procedure is the process of preparing and helping a patient to leave the hospital after treatment and recovery. It ensures that the patient continues proper care at home and prevents complications or readmission. MS. ANEETA SHARMA MSC TUTOR(CHILD HEALTH NURSING )

DEFINITION Discharge of patients from the hospital means , relieving a person hospital setting ,who admitted as an inpatient in that hospital . OR Discharge procedure is the process of preparing and helping a patient to leave the hospital after recovery or completion of treatment, ensuring continuity of care at home and proper follow-up. MS. PURPOSE To ensure continuity of care at home. To provide instructions about diet, medication, exercise, and follow-up. To prepare the patient and family for self-care after leaving the hospital. To prevent complications after discharge. To maintain proper documentation and hospital records. To ensure psychological readiness of the patient for discharge. TYPES OF DISCHARGE MS. ANEETA SHARMA MSC TUTOR(CHILD HEALTH NURSING )

1-PLANNED DISCHARGE:- When the patient has recovered or improved and is ready to go home with the doctor’s approval. Example: A post-surgery patient goes home after complete recovery MS. ANEETA SHARMA MSC TUTOR(CHILD HEALTH NURSING )

2-LAMA (Leave Against Medical Advice):- When the patient leaves the hospital without the doctor’s permission, against medical advice. Example: A patient insists on going home even though treatment is not complete

3-Absconded Discharge:- When the patient leaves the hospital secretly or without informing the hospital authorities. Example: A patient runs away without notice or clearance MS. ANEETA SHARMA MSC TUTOR(CHILD HEALTH NURSING )

4-Transfer Discharge:- When the patient is transferred to another hospital for specialized or advanced treatment. Example: A heart patient sent to a cardiac hospital for surgery

5-Death Discharge:- When the patient dies in the hospital , and the body is handed over to the relatives after completing all formalities EXAMPLE- A patient admitted in the ICU with a severe heart attack passes away during treatment. After confirming death and completing all hospital and legal formalities, the body is handed over to the relatives — this is called a Death Discharge . MS. 6-Partial / Parole Discharge:- When the patient is temporarily allowed to go home for a short period, with the doctor’s permission, and is expected to return. Commonly used for psychiatric patients to test adjustment in the community. EXAMPLE- A psychiatric patient who has shown good improvement after therapy is allowed to go home for one week to see how well they adjust to normal family life. The patient must return to the hospital on a fixed date for follow-up evaluation or continuation of treatment.

DISCHARGE PROCEDURE 1. Checks the doctor's written order for discharge 2. Informs the patient and relatives about discharge 3. Obtains the discharge prescription for medications that have to be continued at home\ 4. Ensures that the unused and unwanted medications are sent to the pharmacy for refund. 5. Cancels the diet and intimates the same to dietary department 6. Documents the relevant information such as date, probable time of departure from the ward, diet taken from hospital & extra facilities used like oxygen, special mattress, special devices, air cooler & fan MS. 7. Arranges the chart in order and send the necessary Documents for billing. Specifies whether the patient has insurance or not \ 8. Checks that the bill has been paid 9. Ensures that the discharge summary from physician is ready. 10. Provides information about follow up care and dietary recommendation regarding home care and medication 11. Removes the identification band 12. Records the time and condition of the patient at departure 13. Ensures that the unit is cleaned and prepared for the next patient MS. NURSING ROLE DURING DISCHARGE 1-Verify Doctor’s Order:- Ensure discharge is approved by the doctor before proceeding. 2-Prepare the Patient:- Make the patient physically comfortable and mentally ready for discharge. Remove IV lines, catheters, or drains as per doctor’s instructions. 3-Provide Health Education:- Explain medications, diet, rest, exercises, wound care, and warning signs. Give instructions for home care and follow-up appointments.

4-Settle Hospital Accounts:- Assist patient/relatives in clearing bills and returning hospital property. 5-Documentation:- Complete discharge summary, nursing notes, and obtain patient/relative signature. 6-Emotional Support:- Reassure patient and family; provide guidance for self-care at home. 7-Escort the Patient:- Help the patient dress, pack belongings, and safely leave the hospital MS.
