The FAST protocol was utilised from the mid to late 90s. The addition of the detection of a pneumothorax was included in the eFAST protocol in 2004.
Where to scan?? RUQ LUQ Subxiphoid / subcostal Suprapubic – longitudinal Suprapubic – transverse Right 3rd to 4th intercoastal space, mid clavicular line, longitudinal Left 3rd to 4th intercostal space, anterior axillary line, longitudinal
B Lines • vertical lines running from the transducer that may extend to the edge of the screen • normal lung there is A-lines are perfectly spaced, and the B-lines are very small
With pneumothorax (a) there is absent sliding lung; (b) A lines may be present, more numerous than normal, and not evenly spaced; (c) B-lines are no longer present
Role of FAST in blunt abdominal injuries Diagnostic modalities Physical exam DPL difficult to perform CT Scan cannot do in pregnant FAST Lack sensitivity
