5. HISTORY HISTORY OF PRESENTING ILLNESS Patient underwent extraction of 46 1 year back due to caries. PAST DENTAL HISTORY Patient underwent scaling 2 weeks back. Patient underwent restoration for 37, 47 2 weeks back. Patient underwent endodontic treatment for 24 and 25, 2 weeks back. Patient underwent endodontic treatment followed fabrication of splinted crowns for 11 and 12 two years ago.

26. Implant placement at 46 region followed by fabrication of implant-supported fixed prosthesis Fixed partial prosthesis replacing 46 using 45 and 47 as abutments. Fabrication of removable prosthesis at 46 region. TREATMENT OPTIONS

27. Proposed Treatment Plan Fabrication of tooth supported fixed prosthesis at 46 by using 45 and 47 tooth as abutments, with the metal crown with porcelain facing for 45, 46 and full metal crown for 47.

28. Treatment plan Maxillary and mandibular diagnostic impressions using alginate and bite registration done using ALU wax . Diagnostic mounting using face bow transfer and interocclusal bite records and mock preparation for 45 and 47 and wax-up. Tooth preparation of 45 and 47 to receive metal crown for 47, Metal crown with ceramic facing for 45 and 46. Final impression after gingival retraction, and provisionalisation with a modified sanitary pontic Permanent FPD cementation with a modified sanitary pontic.
