LABETALOL
Classification Non-selective beta blocker (beta1+beta2) with alpha blocking property.
MOA It blocks beta1, beta2 and alpha1 sympathetic receptors. When given orally alpha to beta blockade ratio is 3:1 When given IV it is 1:7 It mainly acts by decreasing peripheral vascular resistance The cardiac output is not affected It has no effect on uteroplacental blood flow
Dosage Oral dose:100-200mg every 8-12 hours ; increasing frequency to 6 hourly till bp controlled Then a maintenance dose of 200-400mg twice daily is given Maximum oral dose:2400mg
Rx of Hypertensive crisis Initial dose 20 mg IV followed by 40-80 mg IV every 10 min till bp is under control It can also be given as an IV infusion: 250 mg in 250 ml of NS given at a rate of 20mg/hr The drip rate is advised to achieve therapeutic response
Contraindications Severe tachycardia Pulmonary hypertension Bronchospasm Allergic disorders
S/E Maternal Postural hypotension Headache Angina Dryness of mouth Fluid retention Jaundice Fetal No teratogenic effect If given in high dose can cause neonatal hypoglycemia It is secreted in breast milk and can cause HR & BP of neonate
