LACTIC ACID PEELS

INTRODUCTION An alpha hydroxy acid. Occurs naturally – Sour milk, yogurt, tomato juice. Largest molecules among AHAs – slower penetration. Less irritation. More hydrating Lunch time procedure – no peeling Good - dry & sensitive skin

MECHANISM OF ACTION Facilitate dissociation of epidermal cells. desquamation of stratum corneum , regeneration. hyaluronic acid in skin – water holding capacity – moisturizing effect. Epidermis - thinned, smoother, improved texture. Reverse – photodamage , wrinkles, brown spots & roughness of skin

FORMULATION pH or pKa - determinant factor for all acids. L.A > pH 3.5 – Cosmetics as a moisturizer. pH < 3.5 in higher concentration – peeling agent 15 – 30 % solution. 30% (pH 2.3) – used for sensitive, dry, oily, acne prone, dehydrated skin. 40% ( pH 2.2) – used for dry, dehydrated, normal skin, perfect for first time peel users

50% (pH 2.0) – wrinkles , all skin types. Pure lactic acid – 92% (pH 3.5) – peeling agent forprofessional use .

GLYCOLIC VS LACTIC L.A – Texture: sticky, yellowish solution G.A - Watery, clear liquid. L.A – expensive, fade hyperpigmentation better, less harsh. If you never got a peel done before – start with L.A then try G.A

COMBINATIONS It is a component of jessener’s peel , Melaspeel KH (see notes down). Claze ( Arginine HCl , Lactic Acid, Kojic Acid, Urea, Arbutin , Citric Acid): Facial rejuvenation & peri -orbital hyperpigmentation , lip lightening, fine wrinkles . Actipeel (Lactic acid 82%): Skin rejuvenation, ageing, melasma and skin lightening. Pyrulac ( Pyruvic Acid 25%, Lactic Acid 25%): photo damaged skin, body pigmentation & superficial wrinkles.

PROCEDURE Pre peel assesment Explain the procedure to the patient Priming Consent form/photographic documentation Application procedure – similar to other AHA peels.

HOW TO APPLY A PEEL Peel application should be in the order: Forehead > cheeks > chin > perioral > periorbital . We do this in this order because peel should be applied from area of thick skin to thin skin . It is thickest In the area of forehead Forehead should be neutralized last.

POST PEEL CARE

Jessner’s peel Lactic acid 29%

ADVANTAGE Large molecular wt – slower penetration – less irritation Used on sensitive skin & Rosacea . L.A – is a humectant – used on dry skin. Useful in improving the barrier function of the skin Naturally occuring in the skin – allergic reactions rare. Less downtime – normal skin 3 – 4 days

DISADVANTAGE Improvement is gradual. Burning & irritation at times. Less used singly as a peeling agent.
