Documentary Requirements for Coach and Assistant Coach Any school (private or public) personnel may be allowed/designated as a coach or assistant coach, provided he/she meets and submits the following documentary requirements:   He/she must be a Filipino citizen ; Certificate of relevant sports training of twenty-four (24) hours for non-combative sports and forty (40) hours for combative sports and gymnastics; Certificate of relevant experience of one (1) year for non-combative sports and two (2) years for combative sports/ gymnastics;

3. Coach Track Record of participation in the division and regional meet ; Educational or professional attainment of at least 2nd-year college, preferably sports-related course; f. Certificate of Membership in any relevant sports association, or License or certifications/accreditation issued by the National/ International Sports Association, authorized organization, or by the DepEd (Division, Region, or Central Office);

7. Coach/Assistant Coach Record (CACR) Date of birth format should be followed as indicated (mm/dd/ yyyy )

8. Coach/Assistant Coach Record (CACR) Coach/Assistant Coach educational qualifications should be reflected on this table.

9. Coach/Assistant Coach Record (CACR) Coach/Assistant Coach/Chaperone sports training attended related to the sporting event that he/she is handling. Starting from the first training followed by recent trainings he/she attended to for the last three (3) years

10. Coach/Assistant Coach Record (CACR) Certificate of Relevant sports training of twenty-four (24) hours for non-combative sports and forty (40) hours for combative sports/gymnastics

11. Coach/Assistant Coach Record (CACR) Sports Track Record/Experience must be supported by certificates of relevant experience of one (1) year for non-combative sports and two (2) years for combative sports/gymnastics; preceded by first athletic meet attended up to the recent.

12. Coach/Assistant Coach Record (CACR) Signed by the Coach/Asst. Coach, Division Sports Officer, and Division AO/SDS

13. MEDICAL CERTIFICATE g. Medical Records (Medical Certificate of Fitness);

14. CERTIFICATE OF EMPLOYMENT h. Employment/Appointment Paper or duly Notarized Contract of Service that shows at least six (6) months of employment/engagement in the public or private school before the Division meets ; and
