PHYSICAL EXAMINATION

GENERAL EXAMINATION Patient was not in respiratory distress Not cachexic Not on O2 No catheter No mobility aids Has a pink cannular but iv line connected

GENERAL EXAMINATION OF THE HAND BY STANDING ON THE RIGHT SIDE OF THE PATIENT No finger clubbing No palmar erythema No janeway lesions No oslars node No nicotin stain No duptrenys contracture No astrexis and flapping tremors Radial pulse present and of full volume No collapsing pulse present.

HEAD and NECK Conjuctiva pallor present , No Jaundice, C entral cyanosis present N o angular stomatis and oral thrush No reased jugular venous pressure and no lymphadenopathy

GENERAL EXAMINATION OF THE CHEST No scars, spider navie , Chest was symmetrical and oval in shape No swelling or masses No traditional tattoos

CARDIOVASICULAR SYSTEM PALPATION Thrills and Heaves were present. Apex beat was present in the 5 th intercostal space Auscultation Closure of aortic valve and pulmonary valve in the 2 nd intercostal space was heard. Closure of tricuspid valve in 4 th intercostal space was heard. Closure of the mitral valve in the 5 th intercostal space was heard

RESPIRATORY SYSTEM `PALPATION Tracheal was centrally located No swelling and masses Tactile fremitus present Percussion Chest was resonant Auscultation Vascular breath sounds present Vocal fremitus present No crackles

GASROINTESTINAL TRACT General examination of the abdomen. Abdomen was mild distended and symmetrical. Abdomen was rising together with respiration. Scares were present. Linea nigra present. Striae gravidarium present. No distended veins. No Caput medusa .

PALPATION LIGHT PALPATION Tenderness in the lower abdomen Right inguinal region , hypogastric region and left inguinal region And umbilical region DEEP PALPATION No masses and organomegaly Fluid thrill not present.

PERCUSSION Tempanic sound heard on percussion Shifting dullness not present No spleenomegaly No hepartomegaly

AUSCULTATION Bowel sounds were present
