ULTIMATE AIM OF APPLICATION OF TENS IS- PAIN RELIEF
PHYSIOLOGICAL AND THERAPEUTIC EFFECTS OF TENS. CONVENTIONAL TENS:- Selectively activate A β fibers (touch related) , without concurrently activating A δ or c fibers (pain related). TENS induced activation of large diameter fibers inhibits the ongoing transmission of nocciceptive information in the spinal cord and reduces central sensitization ( by activating inhibitory interneuron).
Due to its high frequency effect mostly given in case of chronic cases.
LOW TENS: Activate small diameter A δ fibers that causes muscle contraction (non painful phasic muscle twitch) Indirect activation of A α fibers. Also activates A β fibers causing segmental analgesia. This causes traffic signal phenomenon and blocks the pain.
Burst pattern of low TENS is more tolerable and comfortable than the low frequency single pulses. This releases the endogenous opioids at PAG and RAPHE nucleus causing descending pain suppression system. The low TENS also activates A δ fibers causing segmental analgesia. Used in acute cases mostly
BRIEF TENS: Activate small diameter A δ fibers by applying tens electrodes over peripheral nerves arising from the site of pain, at an intensity tolerable to the patients. High frequency of the current doesn’t cause muscle contraction. A δ fibers activity causes peripheral nerve to block nocciceptive impulse and thus causes pain relief.
A β fibers are also activated causing segmental analgesia. As the intense TENS acts as counter irritant , it should be given for short time. Useful in minor surgical procedures and wound dressing and suture removal.
