Thank you for your request to release loan information to a third party.

Please note that your loan information is confidential and PNC Mortgage,
a division of PNC Bank, National Association requires
your signed authorization in order to release information to a third
party. Please indicate below any information that you are authorizing
us to release.

___ General mortgage loan information, such as: loan balance, next
payment due date, payment options, hardship assistance, payment
arrangements, interest rate, loan term, payoff information.

___ Update contact information, such as: mailing address, telephone
number or email address.

___ Update financial institution information, for example: bank name,
checking or savings account information.

___ Loan documentation, such as: copies of correspondence or signed
documents from the loan closing.

___ Information concerning your escrow refunds, for example: the
status of an escrow check or copies of cancelled escrow checks.

Once you have selected the information you wish to be released,
sign the form below and return to PNC Mortgage via mail or fax:
