Introduction
•Pain is an unpleasant sensory and emotional experience associated with
actual or potential tissue damage or described in terms of such damage.
•The premature pain profile is a comprehensive assessment tool used for new
born babies born either at full term or preterm

When to assess pain
•Performs pain assessment and documents in SALAMA
•at the time of admission
•At the time of painful procedures
•post procedure
•post pharmacological and non-pharmacological interventions
•At the time of transfer and discharge.

Assessment of pain
•Pain score
A new born infant’s pain intensity is determined by measuring:
*the child’s gestational age at birth,
*one behavioural indicator (sleep/awake state),
*three facial expressions-brow bulge, eye squeeze, naso labial fold,
*two physiological indicators-heart rate, oxygen saturation.
These seven indicators are scored together to provide an indication of an acute pain.

Guide To Facial Actions
•Observe the infant for 30 seconds
immediately following the painful event.

Management
•Non Pharmacological
•pacifier with sucrose
•Non Nutritive sucking
•Position or touch
•Environmental modification
•Distraction
•Pharmacological
•Inj. Fentanyl
•Adol syrup
•Syp Chloral Hydrate
•Emla cream
•Inj. Morphine

When to reassess pain
•For oral rectal and IM medications: 30-45 minutes after medications
•IV medications: 15- 20minutes after medications.
•Reassess pain after 30 minutes to 1 hour for non-pharmacological pain
measures to alleviate pain.

Post operative pain assessment
•Assesses Postoperative pain as follows
•Every 15 minutes for 1 hour
•Every half an hour for 2 hours
•Hourly for 4 hours
•Thereafter 4 hourly till discharge
•Reassess for Newborn after
circumcision as follows
*Within 30 minutes of receiving
from OT
*If pain score is above the
acceptable level after the appropriate
interventions, reassess after 1 hour
followed by once a shift as per
routine

Conclusion
•Effective pain management is desirable standard of care for
preterm and term new born and may potentially improve their
clinical and neuro developmental outcomes .
