Introduction Teledentistry is the use of health information technology and telecommunications for oral care, consultation, education and public awareness with the broad goal of improving oral health its applications have been steadily increasing with many branches of telemedicine f uture applications have the possibility of increasing utilization of oral care services, decreasing financial and human costs and improving health outcomes 3
What is Teledentistry? Teledentistry is the use of information technology and telecommunications for oral care, consultation, education, and public awareness in the same manner as telehealth the use of mobile devices like smartphones, electronic health records and portable radiography for the same purposes used in generalist and specialty practices e.g., orthodontics, endodontics, oral surgery, periodontics and dental public health its greatest value is the potential to reduce healthcare inequalities, providing greater access to specialists and timely oral care 4
Definition of Telehealth Delivery of healthcare and the exchange of healthcare information across distances The prefix ‘tele’ derives from the Greek for ‘at a distance’ Telehealth encompasses the whole range of medical activities including diagnosis, treatment, monitoring and prevention of disease, continuing education of healthcare providers and consumers, and research and evaluation Telehealth is designed to assist with case management and monitoring to improve both management of diseases and access to care 5
Methods of Teleconsultation Teleconsultation through Teledentistry take place in three ways; “Real Time Consultation” involves a videoconference in which dental professionals and their patients communicate with one another “Store and Forward Method” involves the exchange of clinical information and static images collected and stored by the dental practitioner for consultation and treatment planning “Remote Monitoring Method” in which patients are monitored at a distance and can either be hospital-based or home-based 6

POTS ( Plain Old Telephone System) Networked programs Point to point connections Primary or speciality care to home connections Home to monitoring center links Web - based e-health patient service sites Delivery Mechanism of Teledentistry 10
Networked Programs Link hospitals and clinics with outlying clinics and community health centers in rural or suburban areas by using dedicated high-speed lines or the internet 11 Point-to-Point Connections Using private networks, used by hospitals and clinics that deliver services directly, or contract out speciality services to independent providers at ambulatory care sites
Primary or Speciality Care to Home Connections Connecting primary care providers, specialists and home health nurses with patients using single line phone video systems for interactive clinical consultations 12 Home to Monitoring Center Links Used for patient monitoring, home care and services that provide care to patients in the home, using normal phone lines and internet Provide direct consumer outreach and services over the internet Under telemedicine, these include sites that provide direct patient care Web-based E-health Patient Service Sites
The medium of communication can be based on Satellite Terrestrial telephone lines Integrated services digital network (ISDN) or Wireless based, and combined technologies Teledentistry can be performed using platforms such as mobile phones, video conferencing, and social media 13

Applications of Teledentistry Teledentistry and dental education Teledentistry can provide: Multipoint interactive continuing education courses Multicentered treatment planning conferences Inter-residency case reviews with community dentists at remote sites Long distance “Hands-on” training to local dentists at remote sites Virtual care groups to provide expanded clinical training Web-based self-instruction educational system 16
Role in Oral Med icine and Diagnosis Virtual video visit to allow for a face to face encounter between the dentist and the patient Ex amination via data transfer of recorded health information (e.g., videos, radiographs, and intraoral photos) Distant diagnosis is an effective alternative in the diagnosis of oral lesions using transmission of digital images by Email Direct Emails can be used for transmission of patient history forms, appointment schedules, referrals, health reports and patient’s concerns and questions Saves patient time and costs associated to transportation use 17
Role in Oral Surgery Assessing patients for dentoalveolar surgery with general anesthesia and nasotracheal intubation are as reliable as those conducted by traditional methods An efficient and cost effective mechanism to provide pre-operative evaluation in situations in which patient transport is difficult or costly Provide fast and clear access to electronically mailed digital images Allows for improved efficiency of the speciality consultation and improved triaging, providing improved care to the maxillofacial patient Assessment of the clinical diagnosis of impacted third molars assisted by the telemedicine equal to the real time assessment of clinical diagnosis 18
Role in Endodontics Can identify root canal orifices based on images of endodontically accessed teeth Can be utilized in the diagnosis of periapical lesions of the front teeth, reducing the cost associated with distant visits and making urgent help available Can identify periapical bone lesions using conventional radiographs Any faults in diagnosis cause complications, problems, waste of time and money 19
20
Role in Orthodontics Interceptive orthodontic treatments provided by sufficiently prepared general dentists and supervised remotely by specialists to reduce the severity of malocclusions Help to reduce the high level of inappropriate orthodontic referrals to consultants Useful in the orthodontic fields, as minor emergencies can be easily at home limiting visits to the dental office to cases of real need Ortho CAD 21
Use of digital 2D and 3D models Measurements and assessment of relationships are done using software that processes images 22
Role in Prosthodontics The use of videoconferencing for diagnosis and treatment planning for patient requiring prosthetic or oral rehabilitation treatment Has the potential to increase the total number of dental specialist services in populated areas CAD/CAM systems Dental crowns, dental inlays, onlays Highest level of computerized support in dental implants placement 23
24
Role in Paediatric Dentistry High quality alternative for children who afraid of dentists Reduce patient’s fear and anxiety compared to clinical examination in real time Use of intraoral cameras- take dental photographs from all angles, with focused illumination Helps in connecting children and their families to needed health and social services 25
As good as visual/tactile examinations for dental caries screening in young children remote diagnosis of children dental problems based on non-invasive photographs constitute a valid resource 26
Role in Community Dentistry Allows dental hygienists to provide oral healthcare to underserved populations by digitally linking up with a distant oral health team Offers a potentially efficient means of screening high risk preschool children for signs of early childhood caries 27

Impact of Teledentistry During Covid-19 As dental treatment involves close inspection, examination, diagnostic and therapeutic interventions of the naso-oro-pharyngeal region, dental professionals are most susceptible to get infected with Coronavirus As pandemic is still occurring, so Teledentistry is becoming an increasing option, proving to be beneficial to both the patients and dentists, for preventable benefit for both of their safety 30
Due to mode of transmission of Covid19 Teledentistry becomes safe environment to perform dentistry Minimized risk of cross-infection Offers a wide range of applications such as remote triaging of the suspected COVID-19 patients for dental treatment Decreasing the unnecessary exposure of healthy or uninfected patients by decreasing their visits to already burdened dental offices and hospitals Although Teledentistry is convenient for restricting the visits, it cannot be avoided emergency visits 31
Benefits and Barrier Benefits Cost efficiency Increase access to dental care Save time for both patient and dentist (need <30 mins) Can prescribe necessary medications prior to treatment appointments Provide 24 hours access to care (beneficial for dental emergencies) Allows patient to avoid missing time at school or work for dental visits Referral for specialized care Dental monitoring 32
Barrier 33 Inaccurate diagnosis, treatment or both Technical problems during data transmission Medi legal and copyright issues have to be considered Inability to perform whole consultation Can’t perform all treatment (emergency surgery) Reimbursement for telemedicine services Privacy concerns
Future of Teledentistry Major trends in Teledentistry Advanced virtual consultations AI and machine learning in dental care Mobile dental health application 34
Predictions for the Future of Teledentistry Integration with other health services Teledentistry is set to become an integral part of holistic healthcare, integrating with other medical services for a more comprehensive approach to health Continued innovation and development The horizon of Teledentistry is limitless 35
Conclusion With all the technological developments taking place in the field of Teledentistry, practitioners may eventually link up to virtual dental health clinics and an entirely new era of dentistry can be created Although Teledentistry is an area of expansion, there are still some barriers to its use 36
