Care: Thalassemia Minor No specific treatment Always Hb 10-13 gm% Do not take any medicines to increase Hb

Why do we want to identify Thal Minor?? Their main role: To be partner in the prevention of birth of any Thalassemia Major Care: Thalassemia Minor

Care: Thalassemia Major Regular blood transfusion Initially 3-weekly and then frequency increases Life-long blood transfusion is not the ideal solution; but the only option for most

Blood transfusion: safe and regular Maintain pretransfusion Hb> 9.5gm% Availability (2 M units per year), Donors Packed cell Filters Safety: NAT tested; Risk of blood borne infections

Control or prevention: Why?? Prevention 10,000 Thalassemia Major new born every year 1,00,000 under care Notice the difference in numbers Long term goal: No child should be born with Thalassemia Major

How to prevent Know your Thalassemia status CBC Hb , MCV, RBC Count Hb 9-12 gm% MCV low: <80 RBC count high:>5 M

HPLC HbA2> 3.5% Encourage: Sibling screening

What to do with report?? Donot Panic You do not have something: which affects longevity or quality of life Pre marital: Check Thalassemia status of partner If Normal: Nothing to be done

Amniocentesis During pregnancy: 6-10 weeks genetic testing Amniocentesis/ chorionic villus sampling If Minor/Unaffected: Nothing to be done

Prevention Chances of Thal Major: 25% If Major: Termination of pregnancy

Can Thalassemia be prevented Small countries like Cyprus, Italy, Greece; No Thalassemia It has been achieved primarily by Pre marital screening
