CONTACT CENTER SERVICES Job Order Slip - is a document received by an organization from an external customer, or another department internal to that organization, describing work to be completed and/or products to be purchased or manufactured.
CONTACT CENTER SERVICES Procedure in accomplishing Job Order Slip 1. Fill out the form by writing the Unit No. and description (name of the equipment). 2. Write your observation about the equipment if it needs to be fixed or replaced. 3. Put the date reported and the name of the person who reported the problem. 4. In activity box, write the resolution in fixing the equipment.
CONTACT CENTER SERVICES Procedure in accomplishing Job Order Slip 5. Write the date when the order was completed. 6. Write in the completed by box the name and signature of the person who fixed or replaced the equipment. 7. List down the name of the parts of equipment replaced or fixed.
CONTACT CENTER SERVICES Borrower’s Slip - is a form used to request for tools and equipment needed for a particular job. It indicates the name of the borrower, the date, the job that is to be done, the person to approve the request, when it was returned, and if it was in good condition.
CONTACT CENTER SERVICES Procedure in accomplishing Borrower’s Slip 1. Fill out the name of the borrower, Student No., Grade & Section, Date and Time Borrowed, and the Laboratory (e.g. Cookery, CCS, CSS, EIM etc.) 2. Write a continuous number in No. column. 3. Write the description (name of equipment) and quantity of needs to be borrowed. 4. On the remark’s column, write the status of the equipment (working, not working, needs replacement, needs to be fixed).
CONTACT CENTER SERVICES Procedure in accomplishing Borrower’s Slip 5. Write the name and signature of the person who borrows the equipment. 6. Make sure to have it approved by the laboratory custodian by writing his/her name and signature. 7. Once the equipment is returned, write the date and time, and make sure to have it signed by the person who received the equipment.
