Objectives : To identify the following : Meaning of triage . Purpose / principle of triage . Triage tool, approach and period . Categories / levels and color code for triage. New innovation and trends in triage.
T r i a g e Statistics demonstrate that deaths as a result of trauma occur in a trimodal distribution: First peak includes victims who die before medical attention can be provided .
T r i a g e Second peak occurs within a few hours after injury commonly referred to as the golden hour that incorporates activation of the emergency medical service (EMS) system, stabilization in the prehospital setting, transportation to a medical facility, rapid resuscitation on arrival in the emergency department, and provision of definitive care for those critically injured.
T r i a g e Third death peak occurs days to weeks after injury as a result of complications , including infection or multiple organ dysfunction syndrome (MODS).
T r i a g e : A process of classifying , categorizing and determining the priority of patient's treatment according to the type and severity of their conditions to get the Right patient to the Right place at the Right time with the Right care provider , using a standard urgency rating scale conducted by specialized nurse. Purpose / principle: Ensure proper initial medical treatment for the right patient at the right time by the right practitioners . Determine the appropriate area for treatment . Avoid misusing and wasting resources and time on unrecoverable or hopeless cases. Decrease mortalit y and morbidit y.
tool , approach and period: An “ across the room assessment” ( A B C D E ) approach is used as the basis tool To identify obvious life threat conditions. Triage process should be completed within 5 minutes.

New innovation and trends: SALT: (sort assess live lifesaving intervention treatment and or transport) was developed as a national all-hazards mass casualty initial triage standard for all patients (e.g., adults, children, and special populations)
New innovation and trends: TST: (ten second triage) i t focuses on the provision of immediate life-saving treatment, such as the control of severe bleeding or the preservation of a casualty’s airway. It does not require any physiological measurement, such as breathing or pulse rate, so it can be used by responders with little or no clinical training. Also, it does not classify casualties as “dead”, but instead categorizes them as “not breathing”.
New innovation and trends: MITT (major incident triage tool) is a new triage for prioritizing patients at the scene of a major incident. Is designed to be rapid, reliable and reproducible. It is based on the Modified Physiological Triage Tool-24, which is featured in the NHS England clinical guidelines for major incidents and mass causality events.
