Loco No 28668 (MU with 28666) Failure date 21/05/2023 Category ELTG Equipment VCB (AAL) Place BNDM/CKP Rly . SER Last M/Sch. TOH=27.05.22/ELIN Last Sch. IB3=20.05.23/DSBM Reason of failure : . While closing DJ, experienced abnormal sound from VCB chamber and DJ could not hold Out station attention : Loco no. 28668 checked at BNDM yard by M/Staff/BNDM and found VCB pressure 4.0 kg/cm 2 . Same adjusted to 4.6 kg/cm 2 by DSBM staff and trouble got rectify. New VCB was fitted at ELIN during TOH on 27-05-22. Shed findings : Loco arrived shed at 18.30 hrs on 12.07.2023 as working. After arrival loco was checked thoroughly and found VCB working normal. It seems that the failure was attributed due to less pressure setting of VCB during IB2 schedule attention at DSBM on dtd. 20.05.2023. History of Equipment : VCB Make: AAL, Sl. No: AAL/04/2022/001N, Mfg: 04/2022, PRV Make: Bechofan . Responsibility : Bad workmanship at DSBM. Letter communicated with DSBM. Action Plan : Nil

Loco No 27964 (MU with 27958) Failure date 23/05/2023 Category ELFG Equipment VCB Place MGPV/WAT Rly . ECoR Last M/Sch. IOH=08.08.22/ELSB Last Sch. IC1=19.04.23/ELIN Reason of failure : While working in train no. N/MUAT/COAL, loco no. 27964 was reported as failed due to VCB not closing and ICDJ. Out station attention : Locos attended on arrival OEC/VSKP by ELS staff of VSKP and found the loco no. 27964 VCB not closing and ICDJ. SO, they adjusted VCB pressure and given fit at 21:00 on 24.05.23. Shed findings : Loco arrived shed at 16.30 hrs on 17.08.2023 as working. After arrival loco was checked and found that loco working normal and VCB operation was checked several times and found OK. The VCB pressure was re- setted by VSKP staff after the incident and loco was working satisfactorily upto a long time after the pressure setting. It seems that the failure was attributed due to malfunctioning of pressure regulator valve of VCB momentarily. On further checking it is found that Shavo make pressure regulator was fitted. Hence it was replaced with Parker make pressure regulator as a safety measure for better reliability of the loco. Then loco offered for further traffic use after schedule attention on 21.08.23. History of Equipment : PRV make: Shavo , Dt. of fitment/OH: 08.08.2022/ELSB. VCB make- Schneider Responsibility : Defective material. Action Plan : Although all Shavo make pressure regulator valves are being replaced with Parker make regulator due to changes in pressure setting automatically of Shavo make pressure regulator valve (HQ action plan).

Loco No 28563 (MU with 28198) Failure date 25/06/2023 Category ELTG Equipment VCB Place GP/CKP Rly . SER Last M/Sch. POH=24.10.22/KGPW Last Sch. IB1=25.04.23/ELIN Reason of failure : . While working in train no. N-KDTR, loco no. 28563 was reported as failed due to VCB insulator burst. Shed findings : Loco arrived shed at 16:15 hrs on 28.06.2023 as dead. On arrival loco checked and found that heavy iron particles (solid) along with dust particles accumulated on VCB insulator and on roof also. VCB removed from loco and thoroughly cleaned then checked in Test Bench i.e. pneumatically checked found OK, VCB analyzer report found OK, high tension testing done, no any abnormality observed. Further, on verbal information through LI, it was learnt that there was dizzling rain at the time of incident also. Due to this, there was temporary leakage path created (due to iron particle accumulation), hence leading to chattering & sparking from VCB insulator (As creepage distance reduced). Loco No 28692 (MU with 28456) Failure date 20/09/2023 Category ELFG Equipment VCB Place DPS/CKP Rly . SER Last M/Sch. TOH=01.10.22/IPTE Last Sch. IB2=18.09.23/IPTE Reason of failure : DJ trips via QPDJ. Out station attention: Loco was checked by SSE/DPS - S.P. Kumar along with CLI - N.C. Biswas . On checking found DDS input of QPDJ feedback signal show low or “0”. Due to digital input of QPDJ feedback not high while BLDJ pressed, as circuit trouble cause DJ trip via QPDJ. Loco attended by shed representative at DPS on 22/09/23 and found DJ trips via QPDJ. Data downloaded and checked found data logged several times as DJ tripped due to QPDJ, error code 1018 (enclosed). So, as a corrective action, QPDJ pressure switch changed and again loco checked found ok. After checking sometimes, loco finally offered for further traffic use Shed findings : Loco arrived shed on 26.09.23 at 09.15 hrs as working. Loco checked found working normal. As a precautionary measure VCB changed. VCB analyser done and all parameters found within limit (Copy enclosed ). Input card and output card of QPDJ pressure switch their also changed and loco kept under observation up to 24 hrs. After than loco checked and found working normal. Therefore, it seems that QPDJ pressure switch was malfunctioning and due to this DJ tripped frequently and loco failed. Thus the failure was attributed due to malfunctioning of QPDJ pressure switch. The said QPDJ pr. switch was last fitted on 19.09.22 during its TOH. History of Equipment : Make: Indfos ; Sl. No. 2672, Mfg: 04/2015, Last O/H:19.09.2022, Fitment date: 19.09.2022 (TOH), VCB make- Schneider. Responsibility : Defects in material. Action Plan : Nil
