
Carrying out electrical work (High Tension / Low
Tension)

Carrying out Industrial Radiography.

Removal of covers from manholes / openings.



Permittee

the concerned supervisor who is executing the
job

Issuing authority

Section In charge or area In charge

6. Permittee identifies the activity
covered under WPS
Permittee sends the request for work permit
in the respective format
Issuing authority visit the work location & assess
whether work can be started. Permits to work by
signing in the work permit form
Work Executed as per the recommendations in work
permit.
Issuing authority inspects the work spot, if situation is
safe, closes the permit.
Copy of permit sent to HSE Engineer
