§The documented nursing care plan should cover preventive actions as necessary

§This can be done in a later period if it is not done initially
§Patient education can be done through booklets/patient information leaflets etc.

The reassessment shall be done by the nursing
professionals in a pre-determined format at
prescribed time.

§The frequency may be different for different areas , eg: patients in ICU and ER
need to be reassessed more frequently
