2. Demographic Details Name-: Saleem Khan Age-: 38 years old Gender-: Male Department-: Genral Medicine DOA-:16/08/2024 DOD-:18/8/2024

Subjective A 38 Year old patient was admitted in the General medicine Ward with chief complaints of , *pain in Umbilical Region *Decrease Appetite from one and half month C/o of Nausea and vomiting from 6 days Past Medical History Pulmonary Tuberculosis -20 Years Back Social History Eating Junk Food from 9-10 Months

4. Objectives CBC Hemoglobin-: 10.2 (13-17)gm/dl PCV-: 32.5 (42-52)% MCV-:77(80-100) fl MCH-:24 (26-34) Pg Amylase Serum Serum Amylase-: [195.0] (40-110) Lipase- Serum Serum Lipase-: [364.0] (10-140)

5. Objectives Radiology –USG (Bulky pancreas) Mild Splenomegaly Left Renal Cyst Prostate Megaly .

6. Assessment Based Upon Subject and objectives Evidence, Case is confirmed as Acute pancreatitis As Patient Diagnostic test shows that patient Serum Amylase and Serum Lipase Level were In Increased along with Low level of Hb , PCV,MCV ,MCH that confirm Acute pancreatitis with anemia

7. Plan Of Treatment According To the patient condition, Patient us advised to take Only liquid Substance During Treatment Treatment Inj Monocef (1gm I.V 12 Hourly) Inj Metro 100 ml I.v Inj pantop 40 mg I.V 24 Hourly Inj Emset 4 mg I.v

8. Patient Counseling Patient Is advised to Avoid junk Food Timely Consume medications Take Light Food and consume liquid Diet Avoid Alcohol Stay Hydrated
