ACL PROTOCOLS

PRE-OP PROTOCOL
•Full extension on affected knee
•Flexion of knee within 10 degrees of normal knee
•Minimal or no swelling
•Full Weight Bearing (FWB)

POST-OP Day 1
•ROM: 0-90 degrees (Dangle side of bed)
•Scooping movement (bed to chair)
•Full Weight Bearing
•Exercises: Quads set, SLR, RKF, AP
•Cold Compress
•If with knee brace: set at 0-90 degrees

POST-OP Day 4
•ROM: 0-110 degrees
•Same exercises as Day 1
•Closed chain
•Gait re-training:
•Full knee extension on heel strike
•Equal strides
•Stairs: Alternating going up, bad leg first going down
•If with brace: 0-120 degrees (unhinged during gait)

POST-OP Day 14
•Full ROM achieved
•Continue Exercises and closed chain
•Stationary bike
•no resistance (1 week): far seat and near seat
•Resistance (50%)
•St

POST-OP: 2nd Month
•Continue full ROM and Exercises
•Continue Bike program
•Weights program:
•High reps, low weight (goal to lift equal to body weight)
•Low Reps, high weight (Goal 2x body weight)

POST-OP 3rd Month
•Full ROM
•Continue exercises, bike, weight program
•Running
•3x a week
•Warm up jog
•Gradual increase in speed
•Goal: 16kph within the month

POST-OP 4th Month
•Full ROM
•Continue Exercises, Bike program, Weight program, and Running
•Trampoline program
•Everyday for 1 month
•5 sets of regular jump = 1 round
•Should be able to do 2 rounds, 3x a day
•Movements dictated by coach

POST-OP 5th month
•Full ROM
•Continue Exercises, Bike program, Weight program, and Running
•Sports-specific exercises
•Vertical jumps
•Long jumps
•Box jumps
•Diagonal jumps
•Lunge walk
•Planting Rice
•Laterals
•Figure of 8
•Obstacle Run

POST-OP 6th month
•Full ROM
•Continue Exercises, Bike program, Weight program, and Running
•May Resume sport with known players until full proprioception
achieved
•Competitive sports may be allowed
