DEFINITION Complementary and alternative medicine (CAM) is the term commonly used to describe a broad range of healing philosophies, approaches and therapies that focus on the whole person including biopsychosocial and spiritual aspect.

DEFINITION ALTERNATIVE THERAPY CAM therapies when used alone referred as alternative therapies. COMPLEMENTARY THERAPIES When CAM therapies are used in combination with other conventional therapies referred as complementary therapies.

Complementary therapy Alternative therapy Used together with conventional medical modalities. Used in place of conventional medicine. An example of a complementary therapy is using meditation to help lessen a patient's discomfort following surgery. i.e.- YOGA,PRANAYAM An example of an alternative therapy is using a special diet to treat cancer instead of undergoing surgery,Radiation, or chemotherapy that has been recommended by a conventional doctor. i.e .- SIDHA, AYURVEDA Difference between Alternative therapies and Complementary therapy

1. ALTERNATIVE MEDICAL SYSTEMS Alternative medical systems are built upon complete systems of theory and practice. Often, these systems have evolved apart from and earlier than the conventional medical approach used.

ACUPUNCTURE Acupuncture is a component of traditional Chinese medicine. It is based on the belief that living beings have a vital energy , called " qi ” that circulates through twelve invisible energy lines known as meridians on the body. Acupuncturists insert needles into specified points along meridian lines to influence the restore balance to the flow of qi . There are over 1,000 acupuncture points on the body

HOW ACUPUNCTURE WORKS There are numerous theories about how acupuncture works. Some of them are: Acupuncture stimulates the release of pain Acupuncture influences the release of neurotransmitters , substances that transmit nerve impulses to the brain Acupuncture influences the autonomic nervous system Acupuncture stimulates circulation Acupuncture influences the electrical currents of the body

AYURVEDA Ayurveda is the traditional medicine of India, which originated there over 5,000 years ago. Ayurveda emphasizes re-establishing balance in the body through diet, lifestyle, exercise, and body cleansing , and on the health of the mind, body, and spirit. According to Ayurveda, everything is composed of five elements : air, water, fire, earth, and space . These elements combine to form the three doshas i.e. Vata kapha pitta

1. THE VATA DOSHA The vata dosha is a combination of space and air . It controls movement and is responsible for basic body processes such as breathing, cell division and circulation. Vata body areas are the large intestine, pelvis, bones, skin, ears, and thighs . People with vata as their main dosha are believed to be quick-thinking, thin, and fast, and are susceptible to anxiety, dry skin, and constipation.

2. THE KAPHA DOSHA The kapha dosha represents the elements of water and earth . Kapha is believed to be responsible for strength, immunity, and growth . Kapha body areas are the chest, lungs, and spinal fluid. People with kapha as their main dosha are thought to be calm, have a solid body frame, and are susceptible to diabetes, obesity, sinus congestion, and gallbladder problems .

3. THE PITHA DOSHA The pitta dosha combines fire and water . It is thought to control hormones and the digestive system. Pitta body areas are the small intestines , stomach, sweat glands, skin, blood, and eyes . People with pitta as their primary dosha are thought to have a fiery personality, oily skin, and are susceptible to heart disease, stomach ulcers, inflammation, heartburn, and arthritis .

UNANI INTRODUCTION Unani medicine is based on the Greece philosophy. It was introduced in India by the Arabs around the eleventh century. Today, India is the one of the leading countries in so for as the practice of Unani medicine is concerned. According to Basic Principle of Unani, the body is a made up of the four basic elements, i.e. Earth, Air, Water, Fire which have different Temperaments, i.e. Cold, Hot, Wet, Dry.

There are four Hormone theory of Hippocrates :  Blood  Phlegm  Yellow bile Black bile The hormones are assigned temperature i.e; blood is hot and moist , phlegm is cold and moist, yellow bile is hot and dry, black bile is cold and dry . As long as hormone are in balance the individual remain healthy. Any imbalance and disturbance in hormones result in disease occurence

DIAGNOSIS The diagnosis of a disease is done by feeling pulse , observation of urine, stool, color of skin and gait etc. TREATMENT The treatment comprises 3 components namely Preventive, Promotive and Curative . Treatment is carried out in 3 forms i.e; Pharmacy therapy, Diet therapy. Regimental Therapy and Surgery. Pharmacy therapy makes the natural drugs mainly herbal but also includes animals, minerals and marine drugs. Regimental therapy is specialty of Unani system of medicine. It has 12 methods for specific and complicated diseases and include methods like cupping ,leeching

SIDDHA INTRODUCTION Siddha System of Medicine in an ancient Science ,which belongs to Dravidian culture. It is very useful in maintenance and restoration of good health. Siddha system accounted for total 4448 disease symptoms and its cure. Thousand of herbs and mineral were Included in Siddha system providing good and easy management of chronic to degenerative, viral to cardiac disease.  The Origin :-Its origin is also traced to mythological sources belonging to the Shiva tradition. According to the tradition , lord Shiva conveyed the knowledge of medicine to his wife Parvati. The knowledge were passed from her to Nandi and finally it was given to Siddhas. Siddha is largely practiced in Tamil nadu

PRINCIPLES OF SIDHA The basic principle of Siddha is that T here is an intimate link between man and environment. The equilibrium of three humors (feces, urine and sweat) is considered health and its disbalance is disease. Based on principle of Triguna-  Vatta  Pitta  Kapha

SIDDHA  Food is the basic building material of the body which gets processed into 3 doshas, 7 dhatus, 3 malas . DIAGNOSIS The diagnosis of disease involve identifying its cause. Causative factors are identified by examination of Pulse ,eyes , color of body , tongue, status of digestive system ,urine and study of voice TREATMENT Treatment based on all the diagnostic character of patient. Treatment takes into account about environment age ,sex ,race , habit , diet ,appetite ,physical and physiological condition . Vaidya has the knowledge of herbs and its effectiveness. Siddha system extensively use drug of vegetable source as well as mineral origin . Use of metal like gold ,silver, sulphur, zinc ,mica etc are only seen in Siddha system of medicine

3. HOMEOPATHY Homeopathic remedies are typically derived from plants, herbs, minerals, or animal products . After being crushed and dissolved in alcohol and/or water, the selected substance undergoes a long process of dilution and succession (a process that involves vigorous shaking of the solution). The solution is then stored.

LIMITATIONS OF HOMEOPATHY 1. A surgical problem which has progressed far beyond its initial stages might not respond to homoeopathy. 2. Some sudden life-threatening situations like heart attacks, paralytic strokes, diabetic comas etc. might have to be initially treated with allopathy, till the patient is out of danger. 3. It is relatively slow acting as compared to the steroids . 4. The success rate is not 100%.

NATUROPATHY Naturopathy uses body's natural healing abilities in prevention and treatment of disease through a healthy lifestyle . It builds immunity, improves mental health, and enhances body functions. Naturopathy mainly focuses on finding the cause of the disease rather than merely treating the symptoms of disease. It uses diet , hydrotherapy , herbal medicine , and other therapies to return the body to a state where it can heal itself.

c . HYPNOSIS Hypnosis is a sort of conscious sleep or trance that can help people deal with addictions, pain or anxiety disorders . In hypnosis, a person is put into an advanced state of relaxation in which he is relatively unaware of his surroundings but not entirely unconscious about it. A hypnotized person follows the instructions given by the hypnotherapist and tends not to be to psycho logic stress and conflict .

d . BIOFEEDBACK Biofeedback is a group of therapeutic procedures that use electronic or electromechanical instruments to measure the process and information to person about their neuromuscular and autonomic nervous system . The information or feedback is given in physical, physiological activity or visual feedback signals. People are trained to exercise some control over the way their bodies work and involves measuring a subject's bodily processes such as blood pressure, heart rate, skin temperature using electronic sensors. Biofeedback helps a person to be more aware of what his body is doing, and to bring reactions under control.

4. MANIPULATIVE AND BODY BASED METHODS Manipulative and body-based methods in complementary and alternative medicine are based on manipulation and/or movement of one or more parts of the body.

a.CHIROPRACTIC Chiropractic is based on the theory that disease conditions result from misalignments of body structures , especially the spine . The treatment for achieving this balance is spinal manipulation to restore correct alignment and full working order. Researches have shown chiropractic to be effective in treating low back pain.

b.OSTEOPATHY Osteopathy is a system of diagnosis and treatment by manipulation that mainly focuses on musculo -skeletal problems . It differs from chiropractic in its underlying theory that it is impairment of blood supply and not nerve supply that leads to problems.

c.MASSAGE THERAPY It is the manipulation of body tissues to promote wellness and reduce pain and stress . It involves a variety of techniques such as stroking or kneading for applying pressure to specific points. Massage therapy may cause side effects and should not be used in people who have infectious or contagious skin diseases

d.REFLEXOLOGY Reflexology is the practice of stimulating points on the feet, hands and ears to improve health or give a beneficial effect on some other parts of the body. It is commonly performed on the particular areas of foot that are believed to correspond to different organs or systems of the body. This helps to eliminate the blockage of energy responsible for pain or disease in the corresponding body part.

5.ENERGY THERAPIES Biofield therapies are intended to affect energy fields that purportedly surround and penetrate the human body. The existence of such fields has not yet been scientifically proven. Some forms of energy therapy manipulate biofields by applying pressure and/or manipulating the body by placing the hands in, or through, these fields.

a . REIKI THERAPY Reiki is a technique, which uses the channeling and flow of energy through the body to improve health and promote healing . It is very much different from conventional healing therapies. Because in a Reiki therapy, there are no medicines or tools needed to heal diseases.

3. BIOELECTROMAGNETIC BASED THERAPY Bioelectromagnetic-based therapies use pulsed energy or magnetic fields to alter the body's electromagnetic fields and cure illness. Magnets have become a popular treatment for various musculoskeletal conditions and even to relieve pain.

MINISTRY OF AYUSH

MINISTRY OF AYUSH It was created in March 1995 as the Department of Indian Systems of Medicine and Homoeopathy (ISM&H). AYUSH received its current name in March 2003. That time it was operated under the Ministry of Health and Family Welfare . The Ministry of AYUSH was formed with effect from 9 November 2014 by elevation of the Department of AYUSH. The Indian Government has decided to introduce International Yoga Award Scheme to promote yoga.

Mainstreaming AYUSH under NRHM The NRHM seeks to revitalize local health traditions and mainstream AYUSH (including manpower and drugs ), to strengthen the Public Health System at all levels. It is decided that AYUSH medications shall be included in the drug kit of ASHA , The additional supply of generic drugs for common ailments at SC/PHC/CHC levels under the Mission shall also include AYUSH formulations. At the CHC level two rooms shall be provided for AYUSH practitioner and pharmacist under the Indian Public Health Standards (IPHS) model. At the same time, single Doctor PHCs shall be upgraded to two Doctor PHCs by inducting AYUSH practitioner at that level.

STRATEGIES : Integrate and mainstream ISM &H in health care delivery system including National Programmes. Encourage and facilitate in setting up of specialty centres and ISM clinics. Facilitate and Strengthen Quality Control Laboratory. Strengthening the Drug Standardization and Research Activities on AYUSH. Develop Advocacy for AYUSH. Establish Sectoral linkages for AYUSH activities Broad Objectives : Mainstreaming of AYUSH in the health care service delivery system to strengthen the existing public health system.

ACTIVITIES : Improving the availability of AYUSH treatment faculties and integrating it with the existing Health Care Service Delivery System. Strengthening of AYUSH Dispensaries with provision of storage equipments . Making provision for AYUSH Drugs at all levels. Establishment of specialized therapy centers in District Head Quarter Hospitals & Medical Colleges. AYUSH doctors to be involved in all National Health Care programmes, especially in the priority areas like IMR,MMR,JSY, Control of Malaria, Filaria, and other communicable diseases etc.

ACTIVITIES : Training of AYUSH doctors in Primary Health Care and NDCP. All AYUSH institutions will be strengthened with necessary infrastructure like building, equipment, manpower etc. One Yoga Therapy Centre will be opened in district Headquarters Hospitals to provide Yogic therapy for specific diseases Block level School Health Programmes to be conducted twice in a year in two groups consisting of 100 students in each group to improve the physical and mental health of the school children.

ROLE OF NURSE IN COMPLEMENTARY AND ALTERNATIVE THERAPY The integrative medicine approach is consistent with the holistic approach nurses are taught to practice. Nurses have the potential for becoming essential participants in this type of health care philosophy. Nurses should be knowledgeable of CAM therapies to make appropriate recommendation.

ROLE OF NURSE IN COMPLEMENTARY AND ALTERNATIVE THERAPY Nurses should also provide advice to client regarding when to seek conventional therapies or CAM therapies. Nurses need to be aware of their state nurse practice act with regard to CAM therapies and practice only with in the scope of these laws. Nurses have knowledge regarding current researches being done in this area to provide accurate information not only to patient but to other health care professionals.

Conclusion The Indian systems of medicine have age old acceptance in the communities in India and in most places they form the first line of treatment in case of common ailments. Of these, Ayurveda is the most ancient medical system with an impressive record of safety and efficacy. We thus observe that the people are shifting their allegiance to the Indian alternative therapies , because these therapies are natural and have no side effects . The western medicine has failed to deal with the chronic disease as well as new antibiotic -resistant bacteria . Ancient therapies,by contrast, have an excellent record of curing the chronic diseases that do not respond well to western medicine .

AYUSH 64, a polyherbal Ayurvedic formulation in Influenza like Illness June 2018 and July 2019 CCRAS, Ministry of AYUSH Abstract Background : Influenza-like illness (ILI) refers to a wide range of viral infections with an important cause of morbidity and mortality worldwide.. In India influenza accounts for 20-42% of monthly acute medical illness hospitalizations during the peak rainy season . AYUSH-64, a poly-herbal drug, is in practice for 40 years for various clinical conditions like fevers, microfilaremia, and inflammatory conditions. Objectives : A study was conducted to evaluate the safety and efficacy of Ayurvedic formulation AYUSH-64 in clinically diagnosed ILI for accelerating the recovery.

Methods : A prospective, nonrandomized,single-center clinical study with pre-test and post-test design was conducted at Raja Ramdeo Anandilal Podar Central Ayurveda Research Institute for Cancer, Mumbai .A total of 38 participants of clinically diagnosed ILI ( 18-65 years) were studied with the one-week intervention of ‘AYUSH 64’ in a dose of 3gm/day and three weeks post-treatment observation period. Assessment of parameters viz . improvement in the symptoms of ILI, frequency of usage of acetaminophen, antihistaminic and cough syrup, hematology, liver function and kidney function tests along-with incidence of secondary complications, and time to return to a normal routine was done. Results : One-week intervention of AYUSH-64 helped to recover from ILI symptoms with reduced frequency of usage of acetaminophen and antihistaminic . The intervention was safe on hematology and biochemical parameters.Noserious adverse effects were observed during the study. Conclusion : AYUSH-64 along-with standard care in ILI is safe and efficacious and this may use in other viral infections with pyrexia as add-on to standard care for early recovery and better outcome

Complementary and alternative therapies for post-caesarean pain Sandra A Zimpel 1, Maria Regina Torloni 2, Gustavo Jm Porfírio 2, Ronald Lg Flumignan 3, Edina Mk da Silva 4 ;2020 SEP Abstract Background : Pain after caesarean sections (CS) can affect the well-being of the mother and her ability with her newborn. Conventional pain-relieving strategies are often under used because of concerns about the adverse maternal and neonatal effects. Selection criteria : Randomised controlled trials ( RCTs ) comparing CAM , alone or associated with other forms of pain relief, versus other treatments or placebo or no treatment , for the treatment of post-CS pain. This included 37 studies (3076 women) which investigated eight different CAM therapies for post-CS pain relief.

RESULT &CONCLUSIONS: Some CAM therapies may help reduce post-CS pain for up to 24 hours . The evidence on adverse events is too uncertain to make any judgements on safety and have no evidence about the longer-term effects on pain. Future trials also need to be large enough to detect effects on clinical outcomes; measure other important outcomes.
