E.H.S Department. SAFETY CONTRACTOR PROGRAM.
The basic objective of PT. INCO Sorowako with respect to
the Safety and Health of contractor’s employees is:
“To eliminate injuries and ill-health by continuously improving workplace
practices and conditions”
It is our policy to ensure that every reasonable precautionis taken to prevent
accidents.
It is your accountability to act with due diligencein all work being performed. If
you are notbeing duly diligent and accident occurs then you may well be
prosecuted, convicted and fined under the occupational Health and Safety Act.

E.H.S Department. Accidents / Incidents.
Unsafe conditions or practices must be reported to your
supervisor immediately.
Any injury, regardless of how slight, must be reported to your
supervisor immediately. Your supervisor will direct you to a
person who is trained to render first aid.
Contractor supervisor and employees may be required to
participate in an investigation of any incident or accident.
The contractor shall ensure that accidents, injuries and “near
miss”incidents which could have resulted in injury or damage to
property are reported to local PT. INCO management (sponsor) as
soon as practicable.

Special Permit are ;
1.Permit to Work
2.Burning Permit
3.Site Clearance certificate.
4.Confined space entry permit.
5.Locking and Tagging Procedure
PERMIT SISTEM.
Is a rule to establish a uniform safe working practice
and procedure to assure that equipment and area are
safe to work on. The system consists of carrying out a
procedure, obtaining special permits and permit to
work.

“Permit to work”
Is an authority issued by authorized employee and is an
assurance that the appropriate precaution has been taken
that it is safe to work on specified equipment subject to
proper precaution being taken by the employee
undertaking the work.
EHS S.O.P No.020 -Rev.01
E.H.S Department.

”Burning and Welding Permit”
Is an authority issued by the authorized department responsible for
the area, and is assurance that necessary precaution has been taken to
prevent predictable fire loss during welding or burning operation is
being performed except in the areas classified as welding shop, or
other areas declared by the department manager as such.
Burning and Welding
Burning and welding are not permitted unless fire protection is on
site is provided by the contractor. Burning permits will be required
for each working shift before commencing any burning and must be
turned in to the appropriate INCO authority at the end of the shift
E.H.S Department.

“Site Clearance Certificate”
Is an assurance that the specified area are clear from
underground installation or detailed warning to carry out
excavation in a recommended manner because of the
definite existence of underground installation
IMPORTANT
This clearance is not an assurance that other underground installation are perfectly void. The dead
electrical-cable and underground piping may stay undetected.
To eliminate the risk of damaging underground installation the engineering blue print has to be
referred to as the main reference in issuing site clearance certificate, and has to be prepared by the
work coordinator.
As a standard practice there has to be warning strip laid and buried along the underground
installations.
E.H.S Department.

Confined Space Entry Permit.
Is an authority issued by authorized employee skilled in gas
detecting system and is assurance that the specified confined space
is safe for entry.
EHS S.O.P 020 -Rev. 01
®Confined space is a space which by design has limited openings for entry and
exit; has unfavorable natural ventilation which could contain or produce
dangerous air contaminants, and which is not intended for continuous
employee occupancy.
®Each Department is to supply a list of confined spaces identified within their
area responsibilities and to develop the safe working procedure how to make
these confined space safety for entry.
®The list of identified confined spaces, the pertinent information on the content
during operation, the suspected contaminants that may exist, and the
procedure to isolate the confined space must be provided.
E.H.S Department.

Follow Standard PT INCO locking and tagging procedures
•obtain permission from PT. INCO to tag
•Sign log book and personal protection tag
•Authorized PT. INCO personnel will operate the switch and give you the only key.
•Attempt to field start the equipment
•Each work group must be removed at the end of each shift
•Personal tags (red) must be removed at the end of each shift
•Status tag must be installed to protect the equipment if work is not complete
•No one can remove another person’s personal red protection tag
•All equipment must be locked and tagged out before working on it
•All sources of energy must be dissipated (ie. Air, water, hydraulic pressure), (Stopped, locked
and tagged out prior to working on any equipment.)
E.H.S Department.
“Locking and Tagging Procedure”.
Is a procedure that has to be carried out by authorized employee
responsible for taking necessary precaution to protect himself and
his party working on or near the specified equipment.
EHS S.O.P 020 -Rev. 01

“Scaffold”
•Inspect before starting work
•No One allowed to ride on rolling scaffold
•Remove or secure equipment before moving
•Make sure handrail and midrail are rigidly
secured
•Access to any area below must be restricted
E.H.S Department.

Fall Protection
Fall arrest conditionis defined as a circumstances in which the work
presents the potential that a free fall may occur. A fall arrest protection system is
then required that stops and suspends a person in the event of a free fall.
Travel Restrain Conditionis defined as a circumstances in which the
work places the worker within 2 meters (6 feet) of a fall hazard. A travel restrain
protection system is required that prevents a person from approaching the fall
hazard thus eliminating the potential for a free fall
Fall Arrest Systemconsist of a full body harness, shock absorber, a lanyard
and an anchor point
Travel Restrain Systemis to be used when a person is working within 2
meters (6 feet) of an “open Hole”. It consists of a safety belt, a lanyard and an
anchor point.
E.H.S Department.

Fire Procedures
•Be familiar with local plant fire procedures
•If a fire occurs, extinguisher or hose is used for any reason.
•Local PT. INCO management must be notified in writing
using the approved form.
•Contractors must provide and maintain fire protection for the
work site
•Burning rubbish will not permitted on site
•Approval must me obtained before any connections are made
to company fire hydrants or water lines.
E.H.S Department.

•Contractors must obey all road signs
•Speed limit in the plants is 30 km/hr. unless otherwise posted
•Pedestrians have the right of way at all crosswalks
•Seat belts are to be worn by all personnel in a motor vehicle
•Seat belts are to be worn on all “ROPS” equipped vehicles
•Operators are responsible for the safe operation of their
vehicle.
