BRONCHODILATORS What is Bronchodilators? These are those drugs which are basically open the airways of lungs to make breather easier. A doctor may prescribe them for asthma and other respiratory conditions .
Classification of Bronchodilators Fig. 1.1. Classification of Bronchodilators
β 2 SYMPATHOMIMETICS Those drugs which copy the action of β 2 receptor antagonist. This drugs given their action why stimulating the sympatommetic system through β 2 receptor. Salbutamol ( Albuterol ) - A highway selective β 2 agonist. It is short acting beta to agonist (SABA) which has a repeat on set of acction (within 5 minutes) and provide relief for 2-4 hours. It is used to abort and terminate attacks of asthma. Dose - 2-4 mg oral,8mg SR tab.,2mg/5ml syrup,100-200 μ g by inhalation. Terbutaline – It is similar to salbutamol in properties and use.
MOA OF SALBUTAMOL AND TERBUTALINE Activation of β -2 adrenergic receptors Activation of adenyl cyclase Increased intracellular concentration of cyclic- 3',5'-adenosine monophosphate (cyclic AMP) Activation of protein kinase A Inhibits phosphorylation of myosin Decreased intracellular ionic calcium concentrations Relaxation of bronchial smooth musle Salbutamol and Terbutaline
DRUG-DRUG INTERACTION The therapeutic efficacy of Salbutamol can be decreased when used in combination with Acebutolol . The risk or severity of hypertension can be increased when Salbutamol is combined with Aceclofenac . The risk or severity of hypertension can be increased when Salbutamol is combined with Acemetacin . Large doses of IV albuterol (not commercially available in the U.S.) and terbutaline sulfate have been reported to cause exacerbation of preexisting diabetes mellitus and ketoacidosis . Salbutamol caused significantly faster elimination of digoxin from the central volume of distribution to deeper compartments .
