CARDIOVASCULAR AND PERIPHERAL ASSESSMENT The assessment includes examination of the heart by inspecting, palpating, and auscultating the landmarks of the heart, and peripheral vascular system examination by assessing the extremities for color , temperature, edema , capillary refills and peripheral pulses.
PURPOSES To determine the sign and symptoms of heart diseases and peripheral vascular diseases.
ARTICLES FOR ASSESSMENT A tray containing: Sphygmomanometer Stethoscope Light source (Torch) Measuring tape Wrist watch Bowl containing spirit swabs Kidney tray Recording reporting chart and pen
PRE PROCEDURAL STEPS Explain the procedure to the patient. Collect the articles at the bed side of patient. Instruct the patient to lie down or sitting position. The light should be adequate. The nurse should perform the examination by standing at the right side of the patient .
INSPECTION Assess the part of the body over heart, which is known as precordium and the lower parts of thorax. Normally there will be no visible pulsation excluding for the apical pulse. Apical pulse will be present at left midclavicular line at 4 th or 5 th intercostal space. Assess the abdominal aorta for pulsation at the epigastric area and at the tip of sternum.

PALPATION It should be performed by using palm of the hand with four fingers. Make sure that the hands are warm before palpation. While during palpation look for any visible pulsation.
