NCW REENLISTMENT CEREMONY CHECKLIST

1. Navy Career Wise Reenlistment Ceremony & Administrative Check List
1. Personal History Sheet (for Reenlisting Officer)
2. Discharge Certificate on Blue Folder with command picture
3. Reenlistment Certificate with Oath of enlistment
4. Spouse / dependents appreciation (if applicable)
5. SRB Check (if applicable, must have an approved message)
6. Pen, plaque (optional) & coin (for CAG or Reenlisting Officer to
hand out if available)
7. Special Liberty cert. (E-6 & below only)
8. MWR Coupon Book or Certificate
9. Reenlistment Contract & Page 13’s (SRB & Leave) from PSD
(pick up by PS1 or CCC the day before reenlistment)
10. Photographer

Suggested Reenlistment Script
1. Reenlisting Officer presents speech (3 minutes max.)
2. Reenlisting Officer: Discharges the member. Optional: CCC
reads Discharge.
3. Member (optional): Comment / Speech (a minute max.)
4. All hands on attention, Reenlisting Officer gives: Reenlistment
Oath. (With Bible: optional)
5. Member & Reenlisting Officer: Signs contract and page 13.
6. Photographer: Photo-shoot (during and after reenlistment and
signing of contract)
7. Reenlisting Officer hands out (if applicable):
a. SRB Check (if applicable, must have an approved message)
b. Special Liberty chit (E-6 & below only)
c. MWR Coupon Book or Certificate
d. Plaque (optional)
8. Photographer: Photo-shoot with member / CO
9. CO (or Reenlisting Officer) hands out command coin (optional)
10. All hands: Congratulations / Cake (optional)
