*Structured Family Caregiving is known as Adult Family Living in Connecticut,Adult Foster Care in Massachusetts and Texas, Monitored In-Home Caregiving (MIHC) in Louisiana,
RIte @ Home in Rhode Island, Shared Living in Ohio, and Structured Family Caregiving in Indiana. CAREGIVER HOMES OVERVIEW
We believe that no one can care for family like family. That is why Caregiver Homes, a division of
Seniorlink, Inc., provides the coaching, support, and financial assistance needed to keep caregiving
at home. We are the leader in Structured Family Caregiving*— an innovative, proven model of
full-time (24/7), in-home care and support for elders and individuals with disabilities.
Caregiver Homes has achieved the highest level of Case Management Accreditation from the
National Committee for Quality Assurance (NCQA) and is the first organization providing Home and
Community Based Services (HCBS) to obtain this distinction.
How Does the Program Work?
Caregiver Homes provides Structured Family
Caregiving on behalf of consumers, caregivers,
states, and managed care organizations.
The credentialed caregiver (usually a family
member) lives with the consumer and
provides around-the-clock care while our care
teams, including a Registered Nurse, provide
professional support using our Assess, Coach,
Collaborate, Document, and Refer (ACCDR)
framework. While the benefit design differs by
state, the model always includes:
yyFull-time (24/7) safety and support
from a live-in caregiver.
yyA daily stipend paid directly
to the caregiver.
yyInsight from the caregiver through
an electronic daily note.
yyFrequent visits from a professional
care team to assess, coach and
support consumer and caregiver.
yyA professional team that pro-actively
communicates with case managers (where
applicable) and other health professionals.
Case Manager Testimonial:
“Caregiver Homes staff kept in frequent contact with me throughout the process to make sure I knew
what the status was at all times ... and my client and her daughter are pleased with the attention
and support they are given by Caregiver Homes staff. The best way I can describe the professional
relationship I have had with Caregiver Homes is seamless.”
