How Does the Program Work?
Caregiver Homes provides Structured Family
Caregiving on behalf of consumers, caregivers,
states, and managed care organizations.
The credentialed caregiver (usually a family
member) lives with the consumer and
provides around-the-clock care while our care
teams, including a Registered Nurse, provide
professional support using our Assess, Coach,
Collaborate, Document, and Refer (ACCDR)
framework. While the benefit design differs by
state, the model always includes:

from a live-in caregiver.
yyA daily stipend paid directly
to the caregiver.
yyInsight from the caregiver through
an electronic daily note.
yyFrequent visits from a professional
care team to assess, coach and
support consumer and caregiver.
