Paediatric surgery Intestinal intusSusception Introduction Symptoms Causes Diagnosis treatment

Introduction Intussusception is a life-threatening illness and occurs when a portion of the intestine folds like a telescope, with one segment slipping inside another segment. This causes an obstruction, preventing the passage of food that is being digested through the intestine. If left untreated, it can cause serious damage to the intestines, intestinal infection, internal bleeding and a severe abdominal infection called peritonitis. Intussusception is the most common cause of intestinal obstruction in children between the ages of three months and six years.

Symptoms The first sign of intussusception in an otherwise healthy infant may be sudden, loud crying caused by belly pain. Infants who have belly pain may pull their knees to their chests when they cry.
The pain of intussusception comes and goes, usually every 15 to 20 minutes at first. These painful episodes last longer and happen more often as time passes.
Other symptoms of intussusception include:
Stool mixed with blood and mucus — sometimes referred to as currant jelly stool because of its appearance.
Vomiting.
A lump in the belly.
Weakness or lack of energy. Diarrhea .

Causes The cause of intussusception is not known, although viral infections may be responsible in some cases. It may be caused by an abnormality (such as a polyp) in the intestines or as a result of recent intestinal surgery.
In most children there is no abnormality present, and the cause is unknown.

Risk Factors Risk factors for intussusception include:
Age. Children — especially young children — are much more likely to develop intussusception than adults are. It’s the most common cause of bowel obstruction in children between the ages of 6 months and 3 years.
Sex. Intussusception more often affects boys.
Irregular intestinal formation at birth. Intestinal malrotation is a condition in which the intestine doesn’t develop or rotate correctly. This increases the risk of intussusception.

Treatment

Enema The pressure of the fluid or air pushes the telescoping bowel back to its normal position, fixing the problem.
There is a chance the intussusception can recur within 24 hours. For that reason, after the bowel is reduced, your child will stay in the hospital for an additional day to ensure there is not a recurrence.
Most of the time the enema will take care of the bowel problem. However, in 10-15 percent of cases, the bowel cannot be reduced and surgery is necessary.

Complications Intussusception can cut off the blood supply to the affected portion of the intestine. If left untreated, lack of blood causes tissue of the intestinal wall to die. Tissue death can lead to a tear in the intestinal wall, called a perforation. This can cause an infection of the lining of the abdominal cavity, known as peritonitis.
Peritonitis is a life-threatening condition that requires immediate medical attention. Symptoms of peritonitis include:
Belly pain.
Swelling in the belly area.
Fever.
Vomiting.
