Patel Age/ Gender :- 69 years/ F Date of Evaluation :- 18/6/2024 Referred by :- Self Chief Complain :- Cough Increase since 1 month, Difficulty in speech & cough Sign and Symptoms :- Allergy Aggrivate in Feb. & March nose itching Vitals :- HR:- 87 bpm BP :- 150/ 90 mm Spo2 :- 97% BMI :- Height / Weight = 149cm/48kg = 21.62

3. Present H/O Patient having difficulty to speech & cough gradually onset before 4 years and than after she visited Iris hospital (ENT) so doctor suggested to visit specialist. That time she was ignored to visit chest specialist and went USA. Since last month, she have speech difficulty and cough got increase so on 17 th June 2024. She visited zydus hospital and she visit our department for PT Treatment. Past H/O :- Fracture in butock region IVD prolapse [5 month] Surgical H/O :- Hysterectomy [26-30 years ago]

4. Personal H/O :- Menopause – 26 years ago Family H/O :- Twin son – Professor at USA Drug H/O :- Anticholestrol, Antihypertensive Medical H/O :- BP [Hypertension] Since -10 years Thyroid – 10 years Investigation H/O :- CT

Subjective Examination Dyspnea:- NA Cough :- Since 3 years, Aggrivates in particular Feb. month Sputum :- NA Pain :- NA Risk Factor :- Modifiable :- Habit :- Smoking : NO Tobacco chewing : NO Alcohol :NO Allergy :- Nose itching / cough Aggrivates in particular month[Feb. and March] Non Modifiable :- Age :- 69 Gender :- Female Hereditary :- NA

Life style disorder :- Hypertension : Since 15 years Thyroid Dysfunction : 10 years On Auscultation :- Lung Sound :- Breath Sound Wheeze Sound [B/L Upper lobe] over Trachea Crackles :- LT Side [Upper lobe] Chest Expansion :-Axillary Level :- 3.5cm Nipple Level :- 3cm Xiphi Sternum :- 1.5cm Muscle tightness :- Not present MMT Normal ROM :- Neck flexion : 55°Rt, Lt) Side flexion : 35° ( Rt , Lt) Rot : 45°( Rt , Lt)
