Chief complaints : Abnormal movements of upper and lower limbs since 1 year. Difficulty in walking since 6 months, Weakness f right lower limb, Aphasia,incontinance of urine. Past medical history : Not known Past medication history : Not known

Physical examination: Vital signs : Temperature : 98 F PR : 78bpm RR : 20cpm BP : 130/80 mm of Hg Systemic examination: CVS : Normal CNS : Abnormal RS : Decreased Dermatology : Normal
DRUG DISCUSSION: 1. Ceftrioxone MOA: interferes with the synthesis of bacterial cell wall Adrs : hypoprothrombinaemia , bleedindg and haemolysis . 2.Pantoprazole MOA: proton pump inhibiter Adrs : Nausea, headach,loose stools,abdominal pain. 3.Ipratropium bromide MOA : Inhalational corticosteroid Adrs :Oral candidiasis , skin bruising. 4 .Rasagiline MOA:MAO-b inhibitor(increases the amount dopamine). Adrs : Dry mouth,constipation and headach . 5.levidopa+carbidopa MOA: Dopamine precursor+peripheral decorboxylase inhibitor Adrs : Dyskinesia .
S.O.A.P NOTES Subjective evaluation: Weakness of right upper and lower limb,aphasia,incontinance of urine. Objective evaluation : EEG and CT Scanning Temprature:98f RR:26 beats/min . Assessment: Drug induced parkinsonism . Plan: Tab . Rasagiline -increases the amount dopamine levels, Tab.levidopa+carbidopa : Dopamine precursor+peripheral decorboxylase inhibitor this increases the dopamine levels by stimulations of dopamine precutsor .
