i want to replace my upper tooth
i extracted my upper tooth before
4 years ago
No abnormalities mentioned with regard to cardiovascular system,
respiratory system, CNS, gastrointestinal system,
genitourinary, endocrine, dermatological system
and musculoskeletal system.
Drug history: no medication used.
No allergies reactions against any allergens.

brushing habits
Scaling and root planning 1 time.
Amalgum filling on UL5.
Extractions without complications.
Not smoker, tobacco user nor alcoholic
Brushing once a day , using soft brush in circular manner
for 1 to 2.5 minutes using signal toothpaste.

Face is symmetrical ( no abnormality detected in the face).
•Lips are competent.
•Normal smile line.
•TMJ: not tender, no clicking, crepitation, deviation,
lateral limitation nor difficulty in mouth opening.
•Lymph nodes: not tender, not palpable nor mobile submental,
submandibular, sublingual and deep cervical lymph nodes.

Hard tissue:
Normal maxilla, mandible, and zygoma.

Soft tissue:
vestibule, buccal mucosa, tongue, floor of the mouth,
palate, frenum are all normal. Gingiva was inflamed.

Oral hygiene: fair
•Plaque: present. Calculus: not present. No malodour.

Shade selection must be done
using Vita classical.
For the temporary crown
And to assess the reduction
tooth preparation
Tooth prepared for full ceramic crown ( zirconia).
Occlusal reduction: on (buccal side ) = 1mm,
and on (lingual side) = 1 mm.

2mm occlusal clearance was done.
Buccal and lingual reduction = 1.5mm with tapered
flat end bur.
Palatal and buccal shoulder finishing line was done
using tapered flat ended bur.
Fine needle bur used in interproximal reduction
(2mm) to avoid damage of adjacent teeth.
Smoothening of the reduction was done using
finishing bur.
Temporary crown was placed after finishing the
reduction.
Secondary impression

First follow up is done after 2 week
Second follow up after 6 weeks then
6 month then yearly
Radiographs should be done
in every recall.
