Subjective evidence :
-c/o Fever, Breathlessness,Since 8 days,cough with
expectoration since 8 days

4. Objective findings:
X-ray: white patches are seen
Rs :--Crepts,Ronchi present
Lab data:
Hb : 11.9gm

5. Assessment:
From the subjective and objective evidence it is
diagnoised as COPD
indication for therapy:
fever,coughwithexpectoration,breathlessness

6. Therapeutic Goals:
Patient Specific:
-Relieve from fever, breathlessness and
cough
Disease Specific:
To eradicate the causitative organism from the
body, to improve the respiratory function.

7. Assessment of previous therapy: Nil
Assessment of current therapy:
Inj ceftriaxone 1000mg iv 1-0-1
O2 inhalation sos
Asthalin+ ipratropium nebulization 4

Tab Paracetomol 650mgoral sos
Syp Terbutaline sulphate 2.5mg+Bromohexyl

8. Planning
Discharge medication:
Pt is still on bed
Monitoring parameters:
Therapeutic parameters:
monitor for the signs and symptoms

9. Patient counseling:
Advice to avoid smoking, and tell about the severity of
disease which helps to take medicines regularly.
Advise the patient to avoid passive smoking, free from
dust, chemicals,irritants and smoke.
Avoid roaming in cold weather
And also advice not to be discontinue the drugs.
If any complaint arise consult the doctor as possible as
early.
Patient is advised to avoid OTC medication
Maintain oral hygienic.
