Clinical & laboratory step in complete denture
Introduction Complete denture is a structure fabricated by dental technologist for usage of the patients which does not have main aspects of ideal oral cavity such as 1- Functional inability of jaws : lack of power in bite or missing teeth which are in charge of bite and even pronouncing words . 2- Aesthetic lack : patients who are in lack of aesthetic aspect of smile , jaw and whole face .
The Clinical and Laboratory Steps of Complete Denture Fabrication Clinical Diagnosis & Treatment plan Primary impression Border moulding and sec. impression Jaw relation / bite block Try in Denture issue Laboratory Pouring of primary impression Special tray fabrication Pouring of secondary impression Bite rim Mounting on articulator Teeth arrangement Demounting, flasking , dewaxing, packing, curing. Finishing and polishing
Primary impression It is an impression in which certain landmarks of the jaw are recorded . Dental materials for impression taking: Alginate Silicon DSA will send to lab for DT pour the impression and fabricate the special tray
Making a special tray Pouring the primary cast using dental plaster. Cast surface should be free of voids. Mark the vestibular depth minus 2 mm with a pencil(for marking the trimming area) Block and fill all of the undercuts in the cast with baseplate wax by melting and applying on the cast slowly . Apply vaseline or likewise material to serve as separating layer . Place a sheet of custom tray and adjust it from the depth to the top to avoid air while adjusting the custom tray material . Trim on the pencil marker that we marked before Make the borders of the tray as smooth as possible Cure the material in trial unit for 2minutes Remove the cast and put the custom tray again in the unit for complete curing
Secondary impression Border moulding using green stick compound Recording the posterior palatal seal. Trimming the excess green stick compound Making the secondary impression using zinc oxide eugenol impression paste DSA will send to lab for DT pour the impression and fabricate the master cast.
Master cast Beading the secondary impression using beading/modelling wax Boxing the secondary impression using boxing or modelling wax Pouring the master cast using dental stone
bite rim Fabricating the temporary denture base using shellac, acrylic or base plate wax. Ideal requirements: Adapt to the tissues Strong and rigid Not reactive with tissues Dimensionally stable Fabricating occlusal rim using modelling wax. The basic factor that should be considered Relationship of natural teeth to the alveolar bone Relationship of the occlusal rim to the edentulous ridge Standart dimensions used to fabricate an occlusal rim Technique of fabrication Clinical guidelines
Jaw relations Recording the tentative jaw relation Vertical jaw relation Centric or horizontal jaw relation Checking the plane of occlusion Orientation relation using face bow Teeth selection
Mounting on articulator Zeroing the articulator Mounting the maxillary cast Mounting the mandibular cast General consideration Teeth arrangement The teeth should be in harmony with the surrounding tissues They should maintain the vertical dimension. They should be efficient for mastication. Posterior teeth should be selected based on function whereas the anterior teeth are selected predominantly based on aesthetics.
Try in Defined as ‘ A preliminary insertion of a removable denture wax-up or a partial denture casting or a finished restoration to determine the fit, aesthetics, maxillomandibular relation’.
Denture processing Preparation of the trial denture Disarticulation Flasking procedure Dewaxing Application of separating medium After try in, the trial dentures are processed in the lab to attain their final form Commonly, denture processing is done using two techniques namely compression moulding and injection moulding . Step in compression moulding technique: Mixing of polimer and monomer Packing Curing Cooling Deflasking
Finishing and polishing Finishing and polishing includes: Trimming Sand papering Pumice wash After finishing, the dentures are stored in water and stored till the day of the issue appointment.
