1. COMMON ERRORS IN DATA COLLECTION FROM QA PERSPECTIVE TRAINING MANUAL

2. 2 Introduction This presentation would address the Frequent Errors flagged by Quality Assurance. These errors are avoidable, and enumerators should pay attention to these common errors when enumerating.

3. 3 Structural Photo Capture photo of entire structure and not just the door, gate or wall.

4. 4 Exchange Structural Photo for Embossed number plate photo Be sure to upload the right photo for the right question.

5. 5 Exchange Structural Photo for Member Photo Be sure to upload the right photo for the right question.

6. 6 Relationship to Head Younger members relate to older ones as Children or Grandchildren Older members relate to younger ones as Parents or Grandparents Be specific about relationship of member to head.

7. Gender and Photo mismatch FEMALE MALE 7 Always make sure gender always matches with Photo

8. Standard Passport Size photo 8 Always crop photo to the right passport size. Ensure that all photos come with white background.

9. 9 Biological Parents alive or dead Biological Parents of members who relate as children must be alive.

10. 10 Household Address and Landmark Use notable landmarks and be specific with names of Landmarks. Eg : N ame of schools and churches. Use adjectives like adjacent, opposite, behind, etc. when describing location of household. Use cardinal points like North, South, East, West. Use descriptions like “100 meters east from the methodist church”

11. 11 Household Address and Landmark Give detailed directions to Households using precise descriptions. Provide house number as it is on the embossed number plate. Provide GPS code as it is on embossed number plate.

12. 12 Main Household Number and Any Other Household Phone number Do not repeat main household number as any other household number. Enter 10 zero’s where there is no other number.

13. Card Information Errors Name on Ghana Card should match with member name. Check auto captured name to make corrections where necessary. Date of Birth on Ghana card should match with date of birth provided. 13

14. Card Information Errors 3 . Double check for Auto captured Names and Ghana card number when photo of card is taken. 4. G hana card number provided should match with number on card. 14

15. Photo of Cards Photos of cards must be clear, sharp and readable. As much as possible keep cards on white background when taking pictures 15

16. Pregnancy and Pregnancy to term Error Number of pregnancies to term and the number of children of female heads. No pregnancies to term means no children. Year of last pregnancy must correspond to year of last child. 16

17. Name of School 17 Provide name of School of members who is currently in school.
