Barriers in communication : Environment as a barrier - Language difference, loud environment Client as a barrier - patient is not able to understand instrutions . - patients don’t listen instruction properly. Doctor as a barrier - Doctor is using tough medical words with patients

Time as a barrier - not giving proper time for instructing a patient because of busy OPD Supporting staff as a barrier - Nursing staff / Pharmacist delivering wrong or incomplete information to patients

Healthcare providers must communicate effectively with the patients: Name of the drug Directions for use Indications Dose Frequency Duration Side effects and Warnings

1. Name of the drug Example : Tablet Pantoprazole 40mg once a day empty stomach The name of the drug should be legible in block letters with the correct dose explained to the patient when and how to take it ?

2. Directions for use Example: Prescribed Chlorhexidine mouthwash Steps : - Before using a mouth rinse, brush and floss teeth - Take proper amount of mouthwash as advised - Dilute it before use - Thirty seconds is the suggested time - Don't rinse, eat or smoke for thirty minutes after using a mouthwash .

3. Indication Example: Rosuvastatin - cholesterol-lowering agent - prevent strokes 4. Dose 5. Frequency 6. Duration 7. Side effects and Warning

Example 1: (MCQ) A patient is taking a drug which is life saving ( eg . chemotherapy) that has known toxic side effects. What will the doctor do? Discontinue the drug at the first sign of toxicity Ensure that complete blood counts are ordered periodically Monitor the function of all organs potentially affected by the drug Teach the patient how to treat the symptoms if they develop

Explanation: Monitor the function of all organs potentially affected by the drug is the answer as the toxicities of each drug may be different hence it is important to monitor the affected organs to keep a watch for the development of side effects

Medical Non-adherence When the patient doesn't take their medication as prescribed Not taking medications as prescribed, is common, dangerous, and costly Mainly due to a lack of communication and regular follow-up

Causes of Medication Non-adherence 1. Forgetfulness 2. Fear and worry 3. Misunderstanding 4. Adverse side effects 5. Complex medication schedule 6. Lack of symptoms and preventive medication 7. Suffering mental health

‘SIMPLE’ Approach to improve Medical Non-Adherence:

Example 2: (MCQ ) A nurse is reviewing a medication administration record before administering medications. Which order should the nurse implement? A. PANTOPRAZOLE ( pantop ) 40 mg OD PO B . PANTOPRAZOLE ( pantop ) 40 mg O D C . PANTOPRAZOLE ( pantop ) 40 mg daily D . PANTOPRAZOLE ( pantop ) 40 mg PO once daily

Explanation: The correct answer is a complete order, it contains the medication, dose, route and time. OD is no longer an accepted abbreviation, it should be written out as "daily" or "every day." The order of “40mg daily" does not specify the route to be used. Answer D is correct answer

Communicate with the patient regarding optimal use of a) drug therapy b) devices c) storage of medicines

Drug Information and Communication to the Patient Disease from which the patient is suffering. - The nature, type, lifestyle changes if needed, prognosis should be explained to the patient. 2. Purpose of drug prescribed - To cure/control/provide palliation/ prophylaxis.

3. Name of the drug prescribed 4. Information about the generic name, amount and expiry of the drug. 5. How to take, when to take, to take at what interval and how long to take? - Dosage form, dose, dosage interval, to be taken with(out) meals etc. should be explained.

6. Information about drug expiry of a drug or formulation 7. Storage of medicines should be informed. 10. Patients should be asked if they have any query

Communication with the patient regarding Storage of Medicines : Medicines , if not stored properly, can lose their efficacy, potency and safety.

The various temperature zones used for medicines are as follows: 1. Cold Storage: 2°C to 8°C 2. Room temperature 3. Any other specific instruction
