Complicaciones mecánicas: La rotura de la pared libre del ventrículo izquierdo L a rotura del tabique interventricular L a insuficiencia mitral aguda

Incidencia y Mortalidad Ocurren a una tasa de aproximadamente 3 por 1000 pacientes con IAM Mayor frecuencia en pacientes con STEMI La más común es la rotura del tabique ventricular en 1 en 1000 La menos común es la rotura de la pared libre La terapia de reperfusión ha reducido la incidencia de estas complicaciones mecánicas Se encontró una tasa de mortalidad hospitalaria del 42,4% después de STEMI y del 18% después de NSTEMI

Los hallazgos fueron diferentes en la fase temprana (<72 horas) y tardía (>4 días). La rotura temprana se caracterizó por un desgarro abrupto en forma de hendidura ( preferencia por los sitios de infarto anteriores) La rotura tardía se caracterizó por la presencia de expansión del infarto. Rotura de la pared libre del ventrículo izquierdo

Factores de riesgo: Sin antecedentes de angina o IM previos Elevación del segmento ST o desarrollo de onda Q en el EKG inicial Pico de CPK-MB por encima de 150 unidades internacionales/L L ocalización anterior del IM E dad >70 años S exo femenino

Presentación clínica: Suele ocurrir dentro de los primeros 5 días después del IM hasta los primeros 90 días posteriores Rotura completa de la pared: hemopericardio Insuficiencia cardiaca choque progresivo taponamiento cardíaco que conduce a muerte R otura incompleta de la pared: trombo + pericardio = sellan pueden progresar a: ruptura franca con taponamiento cardíaco y compromiso hemodinámico,

Diagnostico: El ecocardiograma generalmente muestra derrame pericárdico con evidencia de compresión de la cámara y también puede mostrar la ruptura misma Resonancia magnética cardiaca en pacientes estables Tratamiento: Pericardocentesis con intervención quirurgica

Rotura de pared interventricular En pacientes el tiempo medio desde el infarto hasta la rotura suele ser de 24 horas, pero puede durar hasta 2 semanas. La rotura se desarrolla en el margen del miocardio necrótico y no necrótico La perforación suele ser única y tamaño varia El tamaño del defecto determina la magnitud de la derivación de izquierda a derecha Sin predisposición por cara del IAM

Factores de riesgo : D año miocárdico extenso M ala circulación colateral del tabique M ayor incidencia en primeros infartos Manifestaciones clínicas: Disnea hipotensión y taquicardia Nuevo soplo cardíaco (áspero, fuerte y holosistólico ) Choque cardiogenico

Diagnóstico: A uscultación de un nuevo soplo cardíaco. E cocardiografía transtorácica Angiografía del ventrículo izquierdo al mostrar la derivación del medio de contraste del ventrículo izquierdo al derecho I serción de un catéter con balón en la arteria pulmonar para documentar la derivación de izquierda a derecha Tratamiento: T écnica de cierre percutáneo con dispositivo de rotura del tabique ventricular

Rotura del musculo papilar Ocasionando insuficiencia mitral Ocure dentro de los primeros 7 días La rotura puede ser parcial (ocurrir en una de las cabezas musculares) o completa.

La rotura del músculo papilar posteromedial ocurre de 6 a 12 veces más frecuentemente. El músculo papilar posteromedial----------art. descendente posterior El músculo papilar anterolateral--------art. descendente anterior izquierda y circunfleja izquierda

M anifestaciones clínicas: A parición aguda de hipotensión y edema pulmonar grave P recordio puede estar hiperactivo y puede haber soplo sistólico Diagnostico: El ecocardiograma generalmente muestra el sitio de rotura del musculo papilar con alteración valvular. Resonancia magnética cardiaca en pacientes estables Tratamiento: Intervención quirúrgica
