Introduction Grahani is described in the context of Agni. Grahani as a disease.

Aetimology The word ‘ Grahani ’ is derived from the root ‘ Graha Upadane ’ means to catch or to get.( Amarakosha ) Ie , the action of holding somethimg firm.

Grahani as a organ It is the seat of Jathara Agni. It is otherwise called as Pittadhara Kala It is so called because it does Annasya Grahana . Function: prevent the entry of undigested food into the Pakwashaya from Amasaya . (A.H.Sa3/50-51)

Grahani – a disease Agni Bala depends on Bala of Grahani and vice versa. If Agni Dushti occurs, may lead to a condition called Grahani Roga ( A.H.Sa 3/53)

Lamghana and Pachana (Ch Chi 15/76) Condition: Sareera Vyapita Ama Lamghana-Upavasam Pachana -with Panchakoladi Shodhana

Chikitsa of Vatika Grahani (Ch Chi 15/77-80) First of all observe for Ama pachana ( Ch.Chi 15/76) Deepana - By Sarpi in Alpamatra After Proper Agni Deepti and proper elimination of Mala, Mutra Continue Snehapana for 2-3 days Swedana Niruha Virechana by Eranda taila+Kshara / Tilvaka Ghritam+Kshara Anuvasana -After Shodhana ,( the Asaya will be clean) if Badha varchas – do Anuvasana with Deepaneeya Vataghna Taila . Repeat Snehana (Ch Chi 15/81)

Examination of Sama & Nirama Mala (Ch Chi 15/94) The Ama mala will sink in the water, but the Pakwa Mala will Float in water. But in conditions of Ati drava , Ati Samghata , Saitya , Kapha dushti , the Mala will sink eventhough it is Pakwa Importance : If Amalalakshana are present then do Pachana .

Treatment of Pittaja Grahani Virechana / Vamana . (Ch Ch 15/122). Agni Deepana by Sarpi , Churna etc- Eg - Tiktaka Ghrita , Chandanadi Ghrita , Nagaradi Churna etc

Treatment of Kaphaja Grahani (Ch Chi15/141-) Vamana . Agni Vardhana by Katu , Amla , Lavana , Kshara dravyas Takrarishta , Madya , Madhukasava , Duralaphasava , Pindasava , Madhu Arishta etc

Treatment of Sannipataja Grahani Panchakarma can be done according to the condition.(CH Ch 15/194) For Agni Deepana various Ghruta , Kshara , Asava , Arishta can be done.

Treatment of Grahani is like that of Ajeerna and in Ama condition do the Amapachana like that in Atisara .(A.H.CH.10/2) Treatment of Ajeerna : Ama-Lamghana Vishtabdha-Swedana Vidagdha – Vamana (A.H.Su.8/27)

In general Snehana , Swedana , Sodhana , Lamghana , Deepana , Churna rayoga , Lavana Kshara , Arisha , Asava , Takra Prayoga , Deepaneeya Sarpi are choice of selection. (Ch Chi 15/196).

Modern view Mal absorption syndrome(MAS ): it is the failure of normal absorption of nutrients from the gut. Types- Generalised malabsorpion &specific malabsorption -Primary &secondary C/F: Steaorrhea,Chronic diarrhoea , abdominal distension, flatulence, anorexia, weight loss,etc Irritable Bowel syndrome (IBS): A functional bowel disorder characterised by abdominal pain, altered bowel habit including constipation and diarhhoea , feeling of incomplete defecation, abdominal disension

In addition to altered bowel habit, IBS patient may dedvelop upper GIT symptoms like chest pain, heart burn , dysphagia , fatigue IBD( Inflammatory Bowel Disease) –refers to Crohn’s disease and Ulcerative colitis. Both are similar and is difficult to differentiate. Ulceraive colitis only involve colon,but Crohn’s disease can involve any part. Crohn’s disease: It is an idiopathic chronic ulcerative IBD, characterised by transmural inflammation affecting the segment of terminal ilium or colon, though any part of GIT may be involved.

Clinical features: abdominal pain, diarrhoea , and weight loss. Ulcerative colitis: Clinical feature: Bloody diarrhoea First attack is more severe followed by relapse and remission. Some times frequent small volume of fluid stool or constipation or pellety stools. In severe cases anorexia, malaise, weight loss and abdominal pain may occur.

Conclusion: of concept of Grahani Grahani is mentioned in two aspects. Agni and Grahani are inter related. Agni Dushti is the main cause of Grahani Roga Maintaining of Agni is the treatment principle of Grahani Roga .
