In typical depressive episodes of all three varieties described below (mild (F32.0), moderate (F32.1), and severe (F32.2 and F32.3)), the individual usually suffers from depressed mood, loss of interest and enjoyment, and reduced energy leading to increased fatiguability and diminished activity. Marked tiredness after only slight effort is common. CRITERIA FOR DIAGNOSING DEPRESSION IN ICD-10
(a)reduced concentration and attention; (b)reduced self-esteem and self-confidence; (c)ideas of guilt and unworthiness (even in a mild type of episode); (d)bleak and pessimistic views of the future; (e)ideas or acts of self-harm or suicide; (f)disturbed sleep (g)diminished appetite. Other common symptoms are:
Depressed mood, loss of interest and enjoyment, and increased fatigability are usually regarded as the most typical symptoms of depression, and at least two of these, plus at least two of the other symptoms described for F32 . Minimum duration of the whole episode is about 2 weeks. Diagnostic guidelines for F32.0 (Mild depressive episode)
At least two of the three most typical symptoms noted for mild depressive episode ( F32.0) should be present, plus at least three (and preferably four) of the other symptoms. Minimum duration of the whole episode is about 2 weeks. Diagnostic guidelines for F32.1 (Moderate depressive episode)
All three of the typical symptoms noted for mild and moderate depressive episodes ( F32.0, F32.1 ) should be present, plus at least four other symptoms, some of which should be of severe intensity. The depressive episode should usually last at least 2 weeks Diagnostic guidelines for severe Depression
Requires 4 or more (of 10) symptoms present for at least 2 weeks. Symptoms must include ≥ 2 1 Depressed mood 2 Loss of interest and enjoyment 3 Reduced energy Plus at least 2 of the following symptoms: SUMMERY
1 Depressed mood 2 Loss of interest and enjoyment 3 Reduced energy leading to: Increased fatigability Diminished activity 4 Reduced concentration and attention 5 Reduced self-esteem and self confidence
6 Ideas of guilt and unworthiness 7 Bleak and pessimistic views of the future 8 Ideas or acts of: Self-harm Suicide 9 Disturbed sleep 10 Diminished appetite
• Depressed mood or a loss of interest or pleasure in daily activities for more than two weeks. • Impaired function: social, occupational, educational . DSM-IV Criteria for Major Depressive Disorder (MDD)
At least 5 of these 9, present nearly every day: 1. Depressed mood or irritable most of the day, nearly every day, as indicated by either subjective report (e.g., feels sad or empty) or observation made by others (e.g., appears tearful). 2. Decreased interest or pleasure in most activities, most of each day 3. Significant weight change (5%) or change in appetite 4. Change in sleep : Insomnia or hypersomnia Specific symptoms
5. Change in activity : Psychomotor agitation or retardation 6. Fatigue or loss of energy 7. Guilt/worthlessness: Feelings of worthlessness or excessive or inappropriate guilt 8. Concentration : diminished ability to think or concentrate, or more indecisiveness 9. Suicidal idea: Thoughts of death or suicide, or has suicide plan
