15) A 20 year old patient comes to you with history of high fever for 4 days. Severe headache, backache, joint pain, pain behind eyes, and bleeding from gums and nose. On examination His temperature is 103 degree Fahrenheit and respiratory rate is 34 per minute and pulse is 94 per minute. Severe sweating, nutritional status appears normal. The patient is alert. A) What is your probable diagnosis? Give reasons B) How will you confirm your diagnosis? C) How will you clinically manage the case 2

Answers A) Diagnosis Dengue haemorrhagic fever because of acute onset, high fever of short duration (which is less than 7 days), severe headache, backache, joint pain, retroorbital pain and bleeding tendencies. B) Confirmation of diagnosis Tourniquet test, if there are >20 petechiae per square inch the test is positive. Thrombocytopenia Increase in haematocrit value by >/=20 percent Rise in IGM/IGG titre NS 1 and NS2 Antigen testing 3

C) Treatment Since there is no treatment, management is only symptomatically and supportive therapy. Those who are able to drink, are passing urine and are otherwise healthy can be managed at home with daily 4
