तत एकमादायद्पवधा त्रधा चतुधाण वा छेदनयत्वा कषायकल्पेन पवपाच्य तथय
कषायथय शुस्क्तमनुष्र्ाां घृताभ्यक्ततालुस्जह्वौष्ठः प्रातः प्रातरुपसेवेत,

❑Pathya-Person should consume milk & rice and ghee in large quantity.
❑Varjya(Avoid)-Walking in sun, excess sexual intercourse, exposure to excess heat, Use

Pharmacodynamics:
Anti-inflammatory activity
•Methanolic, ethanolic, chloroform, ethyl acetate and
petroleum ether extracts of fruits of SA proved to have
anti-inflammatory activity in albino rats. The extract
showed significant anti-inflammatory activity
comparable to the reference standard aspirin
Antiatherogenic effect •Semecarpusanacardium(SA) shows antioxidant
property. It has capacity to scavenge the superoxide
and hydroxyl radicals at low concentrations. The
process of atherogenesis initiated by peroxidation of
lipids in low-density lipoproteins was also found
inhibited by SA

•Nuts of SA, extracted with milk,provedmainly for its locomotor and
nootropic activities in different experimental animal models.
Nootropic activity:
Antimicrobial activity•Alcoholic extract of dry nuts of SA (Bhallatak) showed
bactericidal activity in vitroagainst gram negative strains
(Escherichia coli, Salmonella typhi) and two gram positive strains
(Staphylococcus aureusand Corynebacterium diphtheriae).
Hypoglycemiceffect •Ethanolic extract of dried nuts of SA is effective in reducing
blood glucose and investigated in both normal (hypoglycemic)
and streptozotocin-induced diabetic (antihyperglycemic) rats.

AntispermatogenicEffect
•Semecarpusanacardiumextract feeding caused
antispermatogeniceffect evidenced by reduction in
numbers of spermatogenic cells and spermatozoa in
male albino rats.
Nephrotoxicity
•Significant alteration in activity levels of marker enzymes of
kidney as well as histological structure leading to nephritis
were observed, indicating renal dysfunctioningin albino rat.
Results exhibited nephrotoxicity inducing potential of SA nut
oil extract

26. Signs & Symptoms:
1) Dermal Exposure –When applied to skin, it causes irritation,
inflammation, vesication formation, pain & itching. The blister
contains serum. The skin lesion resembles a contusion. Later on, the
lesion turned into an ulcer with sloughing of skin.
2) Ingestion –causes
i) Blister formation in & around oral cavity
ii) Vomiting
iii) Diarrhea
iv) Abdominal pain
v) Hypotension / shock
vi) Delirium

27. When manifestation of toxicity observed,
Internal:
❑Medication of Bhallatakais stopped and white albumen of coconut or juice of
leaves of Chincha(Tamarindus indica)
❑Sesamum or coconut is given to eat.
Local application:
❑Local application of paste of tila(sesamum) rubbed with buffalo milk and mixed
with butter
❑Local application of paste of Yashtimadhu(Glycyrrhiza glabra) and
tila(seasamum) rubbed with milk
❑Local application of rubbed Shalapatra(Desmodiungangetictum).

Allopathic management:

29. ➢The specific antidote for the toxicity of Bhallatakais Bibhitaka(Terminaliabelerica).
➢The decoction or powder preparations with fruit rind and bark of Terminalia belerica
are effective for the sudden reactions and for systemic effects.
➢Drugs that mitigate Pitta like milk and clarified butter and other drugs with cold
potency may also be used
.

30. Five cases of contact dermatitis caused during different stages of shodhanasanskaraof
Bhallatakafruits due to improper handling of utensils and disposal of media used in shodhana
procedure.
Tx: (advised till lakshonopashama–yeidedresults in 20 days)
▪The affected persons were advised external application with pounded Nimba(Azadirachta
indica) leaves on affected parts
▪Internal administration of
Sarivadyasava30 ml thrice daily after food
Triphalachurna5gm before food twice daily.

Case report:

31. Medicolegal Aspect:
i) The juice is used as an abortifacient by application to OS uteri by means of an
abortion stick.
ii) It is applied to genitals as punishment for adultery.
iii) It is used by malingers to produce an artificial bruise to support a false charge.
iv) The juice may be instilled into eyes to produce conjunctivitis (vitriolage)
v) Accidental poisoning may occur from internal administration by quack.
vi) The juice like vitriol has been thrown on the face with evil intentions.
vii) Suicidal & Homicidal use is very uncommon.
