NURSING ASSESSMENT Assess the amount of bleeding per vaginal, onset, duration, color etc. Assess the exposure to risk factors. Assess the onset, level, duration and frequency of pain. Collect the complete history of patient also about the family history. Assess for other signs and symptoms and collect the reports of previous investigations.
NURSING DIAGNOSIS Acute pain related to DUB (Dysfunctional Uterine Bleeding) as evidenced by pain rating scale. Fatigue related to excessive blood loss as evidenced by verbalisation of patient. Risk for deficient fluid volume related to heavy bleeding as evidenced by physical examination. Fear and anxiety related to abnormal health status as evidenced by asking questions from patient.
