A 55 years old fe male presented with c/o breathlessness ,NYHA Class 4 ,Acute in onset past 2 days c/o palpitations past 2 days c / O giddiness past 1 day H/O B/L leg swelling + No H/O chestpain , No H/O reduced urine output , No H/O fever,cold,cough No H/O headache,altered sensorium No H/O trauma Past History: K/C/O DIABETES ON OHAs

3. On examination: Patient conscious Oriented Afebrile No pallor No icterus B/L pedal edema + Dyspneic and Tachypneic S/E: CVS:S1,S2+,no murmur RS:NVBS+,no added sounds P/A:Soft,BS+ CNS:Conscious Moves all 4 limbs

4. Vitals: BP:110/70mmHg measured in right upper arm 120/80mmHg in left upper arm 120/80mmHg in right lower limb 110/70mmHg in left lower limb PR: 123/ min,normal volume and character,regular rhythm,no radioradial and radiofemoral delay . SpO2:98% RA

5. Investigations: CBC: RBS:86 WBC:8,100. UREA:36 RBC:4,19,000. CREATININE:0.8 HB: 12.2 LFT WNL HCT:34.6. D DIMER 1265.24 ng/ml MCV:82.6 USG Normal study MCH:24.6 CHEST XRAY increased brochovascular MCHC:29.8 . markings PLT:3,37,000 .
