The Legal Mandate

Management of Preventions Experience shows that committees formed in companies, even if they fully applied the legal mandate, have not necessarily been as successful as expected. Important : The requirements of the law are considered as a minimum requirements. OSH Management System

INCIDENT INVESTIGATION

A. Model of Accident Causation

Incident Investigation Process Flow

What is an Accident Investigation? Accident investigation is a process of systematic collection (KUMPUL) and analysis of information (ANALISA) relating to an accident investigation. Team Team Selection

Guide for Investigation Team Appointing a team chairperson to determine the direction of an investigation, assign tasks, set deadlines and communicate with upper management. Decide when you will do the various necessary tasks such as interview witnesses or walk the site. Set up system for storing all investigation information and keeping it safe. Set up your ‘investigation headquarters’ right away. Make sure that everyone feels able to input information.
