Significado de cada Onda del EKG Segmento PR : conducción a través del nodo SN  AV  Has de Hiz  Fibras de Purkinje Intervalo PR: Indican la CAPACIDAD DE CONDUCCIÓN del corazón. = Onda P + Segmento PR - 0.12 – 1.20 seg = 120 A 200 milisegundo = 3 – 5 - Si dura MÁS de 200 mseg  BLOQUEO AV 1er grado - Si dura MENOS de 120 mseg  Pre-excitación ventricular Onda Q : Despolarización del tabique Interventricular. - Pequeña onDa NEGATIVA en DII  Se despolariza de Izquierda a Derecha = se aleja de la derivación DII Onda R : Despolarización de la masa ventricular (se ve reflaja más que todo la despolarización del Viz ya que es el más grande. - Positiva en D II - Más positiva en V5 y Negativa en V1 Onda S : Despolarización de la Base del los ventrículos - Isobifásica en DII  . Se despolarizan de ABAJO hacia Arriba por la distribución de las Fibras de Purkinje Complejo QRS: Despolarización ventricular . INTERVALO R-R = FC .

Significado de cada Onda del EKG Punto J :  es el punto donde el complejo QRS se une al comienzo del segmento ST. Representa el final aproximado de la despolarización y el comienzo de la repolarización Segmento ST : Desde el punto J a la Onda T -  siempre debe estar sobre la linea isoelectrica  ONDA T: Repolarizacion Ventricular  - Positiva en DII I n tervalo QT: Comprende todo el proceso electrico ventricular  No mayor a 440 milisegundos = 0,44 Seg  Se aprecia mejor en DII y V5 

P positiva DI y DII  P negativa en AVR  P seguida de un QRS  Que los R-R sean regulares   RITMO SINUSAL NORMAL R-R sean regulares   Que los P-P sean regulares   FC de 60/100 lpm    Intervalo PR >0,12seg  

DATOS IMPORATNTES DEL QT: CORREGIDO Y CONDICIONES QUE LO ALTERAN QT: representa el tiempo que tarda la despolarizacion y repolarizacion ventricular QTc= QTmedido / RR  QT prolongado  

E sto se realiza calculando el intervalo QT corregido con la formula de Bazett ( dividir el intervalo QT entre la raíz cuadrada del intervalo RR) El intervalo QT corregido es normal entre 340 ms y 450 ms en adultos jóvenes (460 ms en menores de 15 años y 470 ms en mujeres adultas ). CORRECIÓN DEL QT 1 3 4 2 El intervalo QT representa la sístole eléctrica ventricular, o sea, el conjunto de la despolarización y la repolarización de los ventrículos. Su medida varía con la frecuencia cardiaca, por lo que es recomendable ajustar su valor a la Frecuencia Cardiaca ¿Cómo se cálcula ? Intervalo QT corto : menor 340 ms, como patológico Intervalo QT alargado: Mayor e 460 ms en niños, 450ms en hombres, 470 en mujeres

TRASTORNOS HIDROELÉCTRICOS Y PATOLOGÍAS QUE ACORTAN EL QT HIPER POTASEMIA HIPER CALCEMIA HIPER MAGNESEMIA PERICARDITIS HIDROELÉCTRICOS PATOLÓGICOS

Patrón juvenil E n ausencia de otros marcadores clínicos o ECG que indiquen enfermedad (IOT inferior y lateral, depresion del segmento ST u ondas Q patológicas), y b) la IOT u onda T bifásica de V1 a V3 en deportistas adolescentes menores de 16 años o prepuberales, más común en mujeres  menores de 30 años   (patrón «juvenil» )  pasa a considerarse hallazgo normal. ONDA T NEGATIVA Menor duración del QRS, S egmento PR más  corto M enores voltajes de las ondas R, S y T en derivaciones precordiales .

TRASTORNO DE DESPOLARIZACIÓN PRECOZ Repolarización temprana hace referencia a hallazgos electrocardiográficos que incluyen la elevación del punto J ≥ 0,1 mV, a menudo en forma lenta o con una muesca, seguida de elevación del segmento ST en 2 o más derivaciones contiguas. Común en pacientes Jóvenes Afrodescendientes de contextura  longilineos , y deportistas. Se aprecia principalmente en las derivaciones de CARA INFERIOR ( DII, D III y AVF) pero también es apreciable en las Antero- Septales (V1 V2 V3 y V4) 

QT DE TIPO CONGENITO: - Síndrome del QT largo congénito (SQTL) 

QT DE TIPO CONGENITO: - Síndrome del QT corto (SQTC)  ▪ Diagnóstico : la frecuencia cardiaca del paciente en cuestión debe ser menor de 130 latidos por minuto, y preferiblemente, menor de 100 latidos por minuto.  ▪ Es de suma importancia en los niños, ya que normalmente, su frecuencia cardiaca es alta incluso en reposo, y el SQTC es una posible causa de Muerte Súbita infantil

¿COMO SACAR LA FC EN RITMO REGULAR E IRREGULAR?   Localice un QRS que se encuentra sobre una línea de división mayor del papel o Contar cuántos cuadros PEQUEÑOS los separa y Finalmente se dividen entre 1500   Ejemplo: 1 cuadro grande = 5 cuadros pequeños, 5 x 3 = 15  1500/ n° cuadros pequeños del intervalo R-R • 1500 / 15= 100 lpm   RITMO REGULAR : REGLA DE LOS 1500

¿COMO SACAR LA FC EN RITMO REGULAR E IRREGULAR?   Contar el número de ondas R en 30 cuadros grandes (6 segundos) y multiplicar x 10  30 cuadros grandes en un ECG representan exactamente seis segundos  Multiplicar tu respuesta por diez te dará el número de latidos que se han producido en un minuto RITMO IRREGULAR:  Es aquel en el que la distancia entre cada intervalo RR no es similar. METODO DE LOS 6 SEGUNDOS. (9 x 10 = 90)
