CONTRAINDICATIONS
/ PRECAUTIONS
⚫Antianxiety drugs are contraindication in
individuals with known hypersensitivity to any of
the drugs within the classification (i.e.
anxiolytics) or group (e.g. benzodiazepines).
⚫They should not be taken in combination with
other CNS depressants and are contraindicated
in pregnancy and lactation , Shock , and coma .
⚫Caution with administering these drug to client
with hepatic dysfunction or sever renal
impairment 7

CLASSIFICATION OF ANTIANXIETY
DRUGS
???Benzodiazepines
???Non benzodiazepines
???Beta blockers
??? (TCAs) Tricyclic anti-depressant drugs
???(SSRIs) selective serotonin reuptake inhibitors
???(MAOIs) Monoamine oxidase inhibitor

BENZODIAZEPINES:
✪The major group of antianxiety drugs for
chronic anxiety
✪First drug of this type (Librium) created in
1957.
✪Mild and transient side effects, Safe drug
when take alone
✪Mechanism: Interact with benzodiazepine
receptors and enhance the effect of GABA
(gamma amino butyric acid )
9

✪D\C gradually due to withdrawal syndrome .
✪Avoid Juices like grapefruit juice do the inhibitor
cytochrome p 450 enzyme so increase
benzodiazepines life .
✪Cross blood – brain barrier lipid soluble .
✪Side effect : CNS: drowsiness , fatigue , decrease
coordination , mental impairment , slow reflex ,
confusion , depression , headache , memory
impairment , over sedation .
???Paradoxical reaction :agitation , change emotion
children and older adult are more risk .

Instruct the client not to drive
or operate dangerous
machinery while taking the
medication .
1 *Drowsiness ,
confusion , lethargy
(most common side
effects)
Instruct the client on long-term
therapy not to quit taking the drug
abruptly . Abrupt withdrawal can be
life – threatening . Symptoms include
depression , insomnia , increased
anxiety , abdominal and muscle
cramps , tremors , vomiting , sweating
, convulsions .
2 *Tolerance
physical &
psychological
dependence (dose
not apply to
buspirone )
1
7

Instruct the client not to drink alcohol
or take other medication that depress
the CNS wile taking this medication.
3 *Ability to potentiate the
effects of other CNS
depressants
Assess the clients mood daily .
Take necessary precautions for
potential suicide.
4 *Possibility of
aggravating symptoms
depressed persons
Monitor lying and standing blood
pressure and pulse every shift.
Instruct the client to arise slowly
from a lying or sitting position.
5 *Orthostatic
hypotension
1
8

Withhold drug and notify the physician.
6 *Paradoxical excitement
(client develops symptoms
opposite of the medications
desired effect )
Have the client take frequent sips of
water , suck on ice ships or hard candy ,
or chew sugarless gum .
7 *Dray mouth

NURSING
CONSIDERATION
❖Monitor vital signs when drug is given IM
or IV , noting hypotension , tachycardia ,
respiratory depression and muscle
weakness
❖Note and report paradoxical reaction like
excitement , muscle spasticity and sleep
disturbances
2
0

❖Warn patient against abrupt withdrawal of
therapy as this may cause vomiting , muscle
cramps and convulsion
❖Advise patient that drug therapy is best
withdraw over several days or weeks unless
urgently needed to stop abruptly
