SECTION 1: FIRE PREVENTION No. Clause Risk 1a) The best time to stop a fire is before it starts. All electrical equipment, machines, and processing equipment, housekeeping conditions and other sources of fire should be checked for fire hazard at regular interval. NEG 1b) Fire extinguisher should be checked regularly to be sure that they ready for any emergency. Each employee must proficient in handling fire extinguisher installed at the area or station where he works. NEG 1c) Gasoline, gases, oil and other volatile liquids with low flash point should be handled with great care. Open flames, lighted cigars/cigarette should be kept away from them. MED 1d) Employees should eliminate, or report to their immediate supervisor or department head/ service area manager, fire hazards, particularly in their work area that may cause the loss of life or destruction of coop property. SECTION 2: HOUSEKEEPING No. Clause Risk 2a) Oil-soaked and paint-saturated rags, papers, waste and other combustible refuse materials should be deposited in non-combustible covered container, having self- closing cover and removed daily from the work areas for proper disposal. LOW 2b) A procedure on safe collection and disposal of all combustible waste and rubbish should be a part of the fire prevention program. LOW 2c) All passageways leading to fire extinguishers and fire exits must be free from obstructions at all times. Fire extinguishers should not be relocated from their officially designated places without previous clearance from Property Development & Logistics Division LOW
SECTION 3: RUBBISH DISPOSAL No. Clause Risk 3a) Combustible rubbish, weeds, grass and other combustible materials or storage drums/tanks of flammable liquids should not be allowed to accumulate in substation yards, in or around pole yards, or near buildings. LOW 3b) Rubbish should be burned only in a designated place away from buildings, sheds, fences and grass or other combustible materials. LOW
SECTION 4: ELECTRICAL EQUIPMENT FIRES No. Clause Risk 4a) Only approved electrical equipment/machine should be used where flammable gases or vapor may be present. LOW 4b) Makeshift wiring should not be used unless absolutely necessary and merely for temporary use. In such case, it must be adequately protected, properly barricaded and should be removed as soon as possible. In no instance should defective or undersize wires be used. LOW 4c) Portable electrical tools and extension cords should be inspected at frequent intervals and repaired or replaced promptly when found defective. Users should visually check them before using. NEG 4d) Employees should not tamper or block circuit breakers, use wrong fuses, or by-pass fuses. LOW 4e) Waterproof cords and sockets should be used in damp places and explosion proof fixtures and lamps should be used in the presence of highly flammable gases and vapors. LOW 4f) Portable lamp bulbs should be protected by heavy lamp guards or by adequately sealed transparent enclosures and kept away from sharp objects and from falling debris. Bare bulb should not be used when exposed to flammable dust and vapors. Lamp bulbs must be considered as potential hazards in area where flammable dust and vapors exist; they must be safe guarded accordingly. LOW
No. Clause Risk 4g) Personally owned electrical heating appliances such as rice cooker, stove, water heater and the like should never be plugged into coop electrical outlets unless duly authorized by the Department Head/Service Area Manager concerned. NEG 4h) All electrical equipment/machine and electrical installation should be periodically inspected and tested to assure continued satisfactory performance and to detect its deficiencies. NEG SECTION 4: ELECTRICAL EQUIPMENT FIRES (Continuation)
No. Clause Risk 5a) In places, where smoking is prohibited, a “ NO SMOKING “ signs should be posted conspicuously. LOW 5b) Where flammable and combustible liquids, vapors, chemicals, gases and the like are stored, hauled and handled, employees should be prohibited from carrying matches, lighters and other spark-producing devices. LOW 5c) Do not place directly a lighted cigar/cigarette butts, matchstick and the like into the waste basket or any place; use ashtrays. LOW SECTION 5: SMOKING
No. Clause Risk 6a) If gasoline, kerosene, LPG (Liquefied Petroleum Gas), acetylene or alcohol torches are used, they should be placed so that the flames are at least 18 inches from wood surfaces. They should not be used in proximity with flammable liquid containers, paper, excelsior or similar materials or in a place where there is presence of flammable vapors LOW SECTION 6: OPEN FLAMES
No. Clause Risk 7a) The transporting of any flammable liquid aboard coop vehicles is prohibited, except permitted or authorized LOW 7b) Coop truck/pick-up which normally carry small amount of gasoline for engine driven equipment such as generator and others shall be permitted to do so, provided, that only approved type container for flammable liquid is utilized and the following requirements are followed. LOW 7b.1) Metal containers of heavy gauge with a maximum capacity of twenty (20) liters should be used. They should be checked for rust and weak spots that may easily fail due to excessive vibration and pressure build-up. LOW 7b.2) Loading is limited to a maximum of two (2) twenty (20) liter containers only. LOW 7b.3) Caps should be tightly closed and provided with rubber seal to prevent vapor leakage. LOW 7b.4) Container should be properly cushioned and secured by the use of wooden racks, ropes and others to prevent metal to metal contact due to vibration while in transport. LOW 7b.5) A well-ventilated truck/pick-up, preferably with open rear-end, should be used. LOW 7b.6) The vehicle should be equipped with at least one (1) unit 10-lb, dry chemical or one (1) unit portable fire extinguisher. LOW 7b.7) Vehicles that are not identified to transport flammable fluids. Materials must seek clearance from the Occupational Health and Safety Officer. LOW SECTION 7: TRANSPORTING FLAME LIQUIDS
No. Clause Risk 8a.1) Fire extinguishers should be maintained in a fully charged and operable condition and kept in their designated places at all times, except when tested, used, repaired or replaced. LOW 8a.2) Fire extinguishers removed from their designated place where they are installed for recharging or repair should be temporarily replaced by spare fire extinguishers of the same type and capacity. LOW 8a.3) Fire extinguishers should be inspected monthly by the Property Development & Logistics Division. (headquarters) and OHSO (Services Areas & Substations) or at more frequent intervals when circumstances require, to ensure they are in their designated places, that they have not been tampered with and fully charged and pressurized and be detect any physical damage, corrosions or other impairments. Fire extinguishers or parts thereof which are not in good operating condition should be immediately recharged, repaired or replaced. LOW 8a.4) Each fire extinguishers should have a durable tag security attached to show the maintenance or recharge date and the initials or signature of the person who performs this service. NEG SECTION 8: PORTABLE AND MANUAL FIRE CONTROL EQUIPMENT 8a.) INSPECTION AND MAINTENANCE
No. Clause Risk 8b.1) Fire extinguishers should not be obstructed or obscured from view. In large rooms and in certain locations where visual obstruction cannot completely avoided the location and intended use of extinguishers should be indicated conspicuously. LOW 8b.2) If fire extinguishers intended for different classes of fire are placed together, the intended application of each should be marked conspicuously to ensure proper choice of extinguishers when needed. LOW 8b.3) In situations where fire extinguishers must be temporarily provided, unless they are of the wheel type, they should be installed on standard portable stands or set on shelves. NEG 8b.4) Each extinguisher should be installed with the operating instructions facing outward and the location should be marked conspicuously. NEG SECTION 8: PORTABLE AND MANUAL FIRE CONTROL EQUIPMENT 8b.) INSTALLATION
No. Clause Risk 8c.1) If, at any time fire extinguisher shows evidence of corrosion or mechanical defect, it should be subjected to a hydrostatic pressure test or replaced. NEG 8c.2) The hydrostatic pressure test should be recorded on a record tag of metal or equally durable material, or a suitable metallized decal, which should affixed (by a heatless process), to the shell of the extinguisher which favorably passes the hydrostatic test. The record tag shall contain the following information: date of test; test pressure; name or initials of persons or agency making the test. NEG 8c.3) For extinguishers subjected to an original factory pressure test of 350 psi or greater, the pressure test should be 75percent of the factory pressure test (as noted on the extinguisher name plate) but in no case less than 300 psi. For extinguishers subjected to an original factory pressure test of less than 350 psi, the pressure test should be 75 percent of the factory pressure test. NEG 8c.4) Extinguisher shells, cartridges or cylinders that show leakage or permanent distortion in excess of specified limits and other defects should be removed from service.
