Concepts and definitions of monitoring and evaluation Monitoring: a systematic continuous collection of data to check on the progress of a program. Evaluation: is a systematic periodic collection of information about activities, characteristics, and outcomes of the program to make judgments about the program. Monitoring & Evaluation is the process of data collection and analysis for informing policy, program planning and project management.

Performance Tracking Systems Is measuring performance against set targets by tracking the progress of the implementation and in achieving results Process monitoring allows for corrective and preventive actions and also for planning and coordination
Major Indicators of AYH programme Major AYH indicators are: Teenage pregnancy rate, Contraceptive prevalence rate among adolescents Number of facilities that give AYH services Mortality rate among adolescents and youth from all causes Pregnancy-related mortality rate HIV prevalence among adolescents and youth
