Topic : Forms and Report
Forms or screen design are used to allow the user to type data into
the system. You can design boxes on the screen where the user can
type in the data.
Criteria for designing a good form for data entry.
1. Attractive,easy and quick to fill in and workable.
2. Provides helpful prompts.
3. Form design should contain headings, instruction and sections.
Command button usually apply in the form.
1. Text box – display message or provide a place for a user to
enter data.
2. Toggle button – used to represent on or off status.
3. List box – displays a list of choices that the usercan select.
4. Drop down list down box – displays the current selection.
When the user clicks the arrow, a list of the available choices
display.
5. Option button / radio button – represents one choice in a set
of options. The user can select only one option at a time.
6. Check box – used to select one or more choices from a group.
Selected options are represented by a check mark or an X.
7. Calendar control – allows the user to select a date that the
system will use as a field value.

Reports are summaries of the data held on a database. They have a
date and a title and contain neat columns of data. Reports are usually
printed on paper.
