INTRODUCTION Humeral fractures traditionally treated nonsurgically , with predictably satisfactoryoutcomes.Strong bias formerly existed against surgica lintervention due to high rate of complications.Both operative and nonoperative treatments have been refined.

ANATOMY Humeral diaphysis extends from the upper borderof the insertion of the pectoralis majorproximally to the supracondylar ridge distallyFracture alignment determined by the location ofthe fracture relative to the major muscleattachments , most notably the pectoralis majorand deltoid attachments

Deforming forces Example of a fracture distal to pectoralis majorattachment and proximal to deltoid tuberosityAdduction of proximal fragment results
