Investing layer of deep cervical fascia By – Gaurav kaushik Roll no. - 31

Deep cervical fascia Deep fascia around the neck is known as ‘‘ DEEP CERVICAL FASCIA OR FASCIA COLLI’’ As it forms collar around the neck. Lies deep to platysma muscle . It present around muscles, blood vessels & organs of the body.

Layers of deep cervical fascia Investing layer of deep cervical fascia Pretracheal fascia 3.Prevertebral fascia Note:-All three continue to form carotid sheath.

Investing layer Deep to platysma muscle Surrounds neck like collar Forms roof of posterior triangle of neck Most superficial fascia of deep fascia

Investing layer split to enclose:– 2 muscles- trapezius & sternocleidomastoid. 2 glands- submandibular & parotid. 2 spaces- suprasternal & Supraclavicular . Roof of 2 triangles – posterior & anterior tringles . Thickened to form- parotido – masseteric fascia & stylomandibular ligament.

Modification in upper part It splits into superficial and deep lamina to enclose parotid gland. Superficial part extend superiorly as Parotid masseteric fascia Deep part extend b/w styloid process & angle of mandible. It is thickened as stylomandibular ligament. Fascia also split near lower border of submandibular gland to enclose it.

Modification near lower part Suprasternal space of Burn’s : Near manubrium at jugular notch , it splits into superficial and deep layers. The space b/w two layers is called as suprasternal space. it contains:- Sternal head of SCM Lower portion of jugular vein Small amount of fat Lymph nodes(sometimes)

Supraclavicular space:– Above middle third of clavicle,fascia also splits. Posterior layer surrounds inferior belly of omohyoid to form intermediate tendon of omohyoid . Deep layer fascia merge around subclavius . It contains:- Terminal part of EJV Supraclavicular nerves

Clinical Q. External jugular vein injury may resulting in an Air embolus , why ? EJV lies superficial to sternocleidomastoid. It drains into subclvian vein after penetrating investing layer & may be kinked at this point. If the proximal are art of vein is damaged , it may be held open by surrounding fascia & air may then be sucked in, resulting in an air embolus Note:-because of presence of thick tough parotido-masseteric fascia which covers parotid gland swellings of parotid gland( mumps,parotitis ) are very painfull .
