Introduction Gr ief is a “normal but bewildering cluster of ordinary human emotions arising in response to a significant loss, intensified and complicated by relationship to the person or object lost.” -Mitchel and Anderson (1983) 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Gri ef cannot be prevented. No two people with same type of loss grieve in same way, it is very personal. Grief work is very hard and requires enormous energy from the griever. It is rarely orderly and predictable, because objects memories and other events can cause surge in feelings of loss , grieve work is never fully completed. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… However, g rief can diminish and healing can occur when the pain of loss is less . 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Definit ion Gr ief is a series of intense physical and psychological responses that that occur following a loss. Loss leads to mourning - the period of time during which the grief is expressed, and resolution and integration of loss occurs. Bereavement is the period of grief following behaviors of a person experiencing loss . 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Cont'd… Grief is also define d as, “ the subjective response of emotional pain to actual or anticipated loss”. It is the total process of reacting and responding to the losses in one's life. Healthy grieving is time limited, becoming less intense as time passed, but takes 1 year or more to resolve fully. It is normal, natural, necessary and adaptive response to a loss. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Normal gri ef Normal gr ief is a common and universal reaction characterized by complex, emotional, cognitive, social, physical , and behavioral and spiritual responces to loss and death. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Some normal feelings of grief are disbelief, yearning, anger and depression. Although manner of death ( violent, unexpected, traumatic) increases risk to the survivors normal grief response, it doesn't always determine how an individual will actually grieve. Helping coping mechanisms for grieving people include hardiness and realisence, a personal sense of control, and the ability to make sense of and identify positive possibilities after a loss. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

A nticipatory grief A person experiences anticipatory grief before the actual loss or death occurs, especially in situations of prolonged or predicted loss such as caring for patient diagnosed with dementia or cancer. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Fa mily members often grieve the impending loss of companionship, control and sense of freedom and mental and physical changes to be experienced by their loved ones. When grief extends over a long period of time, people absorb loss gradually and prepare for its inevitability. They often experience intense responses to grief (eg:- shock, denial and tearfulness) before the actual death occurs and often feel relief when it finally happens. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Another way to thi nk about anticipatory grief is that its a forewarning or cushion that gives people time to prepare or complete the tasks related to the impending death. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Disenfranchised grief P eople experience disenfranchised grief when their relationship to the deceased person is not socially sanctioned, cannot be shared openly, or seems of lesser significance. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… The person’s loss and gr ief do not meet the norms of the grief acknowledgedged by his/her culture, thereby cutting the grieving person off from social support and sympathy given to people with more acceptable losses. Examples include; death of a former spouse, a married lover, death of a patient, miscarriage, etc. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Sometimes people ex perience losses that are marked by uncertainty. Ambiguous loss , is a type of disenfranchised grief, can occur when the lost person is physically present but not psychologically available, as in cases of severe dementia and brain injury. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Complicated gri ef Some people do not experience a n ormal process. In complicated grief, a person has a prolonged or significantly difficult time in moving forward after a loss. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… He or she experiences a chronic and disruptive yearning for the deceased; has trouble in accepting the death and trusting others and feels excessively bitter, emotionally numb or anxious about the future. Complicated grief occurs more often when a person had a conflicted relationship with the deceased, prior or multiple losses or stressors, mental health issues or lack of social support. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Loss ass ociated with homicide, suicide, sudden accidents or death of a child has the potential to become complicated. Specific types of complicated grief include, chronic, exaggerated, delayed and masked grief. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Other types Chronic grief A person with chronic grief experiences a normal grief response, except that it extends for a longer period of time. This can include years to decades of intense grieving. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… 2. Exaggerated grief A person with exaggerated grief response often exhibits self - destructive or maladaptive behaviour, obsessions or psychiatric disorders. Suicide is a risk for these individuals. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… 3. Delayed grief Delayed grief response is unusually delayed or postponed because the loss is so overwhelming that the person must avoid a full realisation of the loss . A delayed grief response is frequently triggered by a second loss, sometimes seemingly not as significant as first loss. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… 4. Masked grief Sometimes grieving person behaves in ways that interfere with normal functioning but is unaware that disruptive behaviour is a result of loss and ineffective grief resolution. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Factors affecting gri ef 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Human development Pati ent's age and stage of development affect the grief response. For example; toddler cannot understand loss or death but often feel anxiety over the loss of their toys and separation from parents. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Common ex pressions of grief include changes in eating and sleeping patterns, bowel and bladder problems, increased fussiness. School age children understand the concepts of loss but not the causes of a loss . Some have intense periods of emotional expression and experience changes in eating, sleeping and level of social engagement. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Y oung adults undergo many necessary developmental losses related to evolving future. They leave home, begin school or a work life, form a significant relationships . In the same way, middle adulthood people are more mature to handle losses but it changes in older adults due to their poor coping skills. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Personal relationship When loss involves another person, the quality and meaning of lost relationship influence the grief response. When a relationship between two people was rewarding and well connected, the survivor often finds it difficult to move forward after the death. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd … Gr ief work is hampered by regret and a sense of unfinished business, especially when people are closely related but did not have a good relationship at time of death. Social support and the ability to accept help from others are crucial variables in recovery loss and grief. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Nature of the loss Exploring the nature of loss will help to understand the effect of the loss on the patient's behaviour, health and wellbeing. This may arise questions like; Was the loss avoidable? Is it permanent or temporary? Is it actual or imagined? 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Encouraging patients to share information about the loss will help you better develop appropriate interventions that meet the individualized need of patients. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Co ping strategies The losses that pat ient face from the time they were children formulate the coping skills they will use when faced with larger and more painful losses in adulthood. These coping strategies such as talking, journaling and sharing their emotions with others maybe healthy and effective. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… They may also be unhealthy and ineffective such as increased use of alcohol, drugs and violence. Nurses provide support by assessing a patient's coping strategies, educating about new and healthy strategies, encouraging use of these strategies. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Socio-ec onomic status It inf luences a person’s grief process in direct and indirect ways . Because of role changes, a newly widowed mom finds herself searching several jobs to make ends meet and doesn't get enough time to initiate self care or allow herself to support healthy grief. Works such as buying a tree plant in honour of deceased or travel to support groups maybe unrealistic. It directly affects the grief process. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Culture During times of losses and gr ief, patients are and families draw on the social and spiritual practices of their culture to find comfort, expressions and meaning in the experience. To provide best care possible, it is necessary for us to ask about cultural beliefs and practices. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Pat ients and families will rarely offer this information without prompting . Expressions of grief in one culture do not always make sense to people from a different culture. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

S piritual and religious beliefs Like cultural inf luences, spirituality and/or religious practices and belief provide a framework to navigate, understand and heal from loss, death and grief. Patient’s faith may influence the way they respond to an illness, treatment, advanced life support options, autopsy, organ donation and what happens to the body and spirit after death. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›

Contd… Caring for patient is a holistic approach, which includes the spirit, ensures that we are providing patients with the best possible individualized care. 9/20/2024 Roshani Kumari Patel , B.Sc. nursing 3rd Year ‹#›
